目的:探究俯卧位通气联合肺复张( RM)对重度急性呼吸窘迫综合征( ARDS)患者肺功能及预后的影响。方法回顾性分析本院2013年01月~2015年01月收治的62例重度ARDS患者临床资料,按治疗方法分为研究组与对照组,每组31例,对照组采用俯卧位通气治疗,研究组在此基础上联合RM治疗,分析两组治疗效果。结果对照组治疗后CI、CVP水平均低于治疗前,研究组治疗后CVP水平低于治疗前( P<0.05),治疗3、5 d的PaO2/FiO2高于对照组,治疗28 d病死及不良事件发生率低于对照组,差异均具统计学意义(P<0.05)。结论俯卧位通气联合RM治疗可显著改善ARDS患者氧合能力及其预后。

목적:탐구부와위통기연합폐복장( RM)대중도급성호흡군박종합정( ARDS)환자폐공능급예후적영향。방법회고성분석본원2013년01월~2015년01월수치적62례중도ARDS환자림상자료,안치료방법분위연구조여대조조,매조31례,대조조채용부와위통기치료,연구조재차기출상연합RM치료,분석량조치료효과。결과대조조치료후CI、CVP수평균저우치료전,연구조치료후CVP수평저우치료전( P<0.05),치료3、5 d적PaO2/FiO2고우대조조,치료28 d병사급불량사건발생솔저우대조조,차이균구통계학의의(P<0.05)。결론부와위통기연합RM치료가현저개선ARDS환자양합능력급기예후。
Objective To explore the effect of prone position ventilation and lung recruitment ( RM) on lung function and prognosis of patients with severe acute respiratory distress syndrome ( ARDS) . Methods The clinical data of 62 patients with severe ARDS from January 2013 to January 2015 were reviewed, and they were divided into the study group and the control group, 31 cases in each group. The control group was given prone position ventila-tion, and the study group was given prone position ventilation plus RM. The curative effect of the two groups was ana-lyzed. Results After the treatment, the levels of CI and CVP decreased in the control group, and the level of CVP decreased in the study group (P<0. 05). The value of PaO2/FiO2 was higher, and the mortality and the incidence of adverse reactions were lower in the study group than in the control group ( P<0. 05 ) . Conclusion Prone position ventilation combined with RM can significantly improve oxygenation and prognosis for patients with ARDS.
