目的：观察局麻镇痛系统和静脉镇痛法对肺癌术后患者免疫功能的影响。方法择期行肺癌根治术患者60例，随机分成A组（静脉镇痛)和B组（局麻镇痛），分别于术前（T1）、术毕（T2）及术后24h（T3）、48h（T4）抽取外周静脉血测定T淋巴细胞亚群及NK细胞水平。结果 A组T2、T3、T4与T1时间点相比，患者免疫细胞数量均下降（P＜0.05）；B组T2与T1、T3、T4时间点相比，患者免疫细胞数量均下降（P＜0.05）；在T4时间点， A组患者的免疫细胞水平较B组明显降低（P＜0.05）。结论局麻镇痛系统对患者术后细胞免疫功能抑制较轻。

목적：관찰국마진통계통화정맥진통법대폐암술후환자면역공능적영향。방법택기행폐암근치술환자60례，수궤분성A조（정맥진통)화B조（국마진통），분별우술전（T1）、술필（T2）급술후24h（T3）、48h（T4）추취외주정맥혈측정T림파세포아군급NK세포수평。결과 A조T2、T3、T4여T1시간점상비，환자면역세포수량균하강（P＜0.05）；B조T2여T1、T3、T4시간점상비，환자면역세포수량균하강（P＜0.05）；재T4시간점， A조환자적면역세포수평교B조명현강저（P＜0.05）。결론국마진통계통대환자술후세포면역공능억제교경。
ObjectiveTo observe the influence of different analgesic methods on cellular immune function in patients after lung surgery.MethodsSixty patients undergoing elective lung cancer surgery were selected.All cases were randomly divided into two groups:group A(intravenous analgesic system) and group B(local anesthesia analgesic system).Peripheral blood samples were respectively obtained before operation(T1),at the end of operation(T2),24h(T3) and 48h(T4) after operation.The T-lymphocyte subsets and natural killer cells(NK) were measured.ResultsIn group A compared to T1 point,the number of immune cells decreased at T2,T3 and T4(P<0.05).In group B compared to T2 point,the number of immune cells decreased at T1,T3 and T4(P<0.05).At T4 piont,the number of immune cells in group A signiifcantly decreased than group B(P<0.05).ConclusionLocal anesthesia analgesic system can reduce the inhibition of cellular immune function in patients after lung surgery.
