目的：探讨右美托咪定复合依托咪酯麻醉对于胃癌根治术患者术后疼痛与感染的影响。方法择期胃癌根治术患者72例根据随机抽签原则分治疗组与对照组各36例，对照组在麻醉诱导中辅助应用依托咪酯，治疗组辅助应用右美托咪定复合依托咪酯。结果2组患者不同时间点的收缩压与舒张压变化对比均无统计学意义，心率都呈现明显下降的趋势（P＜0.05），不过治疗组用药时与拔管时的心率都高于对照组（P＜0.05）。2组的恢复自主呼吸时间与拔除气管导管时间对比差异无统计学意义（P＞0.05）。治疗组的术后疼痛与感染评分都明显少于对照组，同时术后疼痛与感染发生率也低于对照组（P＜0.05）。治疗组术后躁动、恶心呕吐、寒战及呼吸抑制等不良反应发生情况明显低于对照组（P＜0.05）。结论右美托咪定符合依托咪酯在于胃癌根治术患者的应用能保持围手术期心率的稳定，减轻术后疼痛与感染的发生，减少苏醒期不良反应的发生，有很好的应用价值。
목적：탐토우미탁미정복합의탁미지마취대우위암근치술환자술후동통여감염적영향。방법택기위암근치술환자72례근거수궤추첨원칙분치료조여대조조각36례，대조조재마취유도중보조응용의탁미지，치료조보조응용우미탁미정복합의탁미지。결과2조환자불동시간점적수축압여서장압변화대비균무통계학의의，심솔도정현명현하강적추세（P＜0.05），불과치료조용약시여발관시적심솔도고우대조조（P＜0.05）。2조적회복자주호흡시간여발제기관도관시간대비차이무통계학의의（P＞0.05）。치료조적술후동통여감염평분도명현소우대조조，동시술후동통여감염발생솔야저우대조조（P＜0.05）。치료조술후조동、악심구토、한전급호흡억제등불량반응발생정황명현저우대조조（P＜0.05）。결론우미탁미정부합의탁미지재우위암근치술환자적응용능보지위수술기심솔적은정，감경술후동통여감염적발생，감소소성기불량반응적발생，유흔호적응용개치。
Objective To investigate the effect of dexmedetomidine and etomidate anesthesia on postoperative pain and infection in patients underwent gastrectomy. Methods Seventy-two patients underwent elective gastrectomy were divided into the treatment group and control group by random drawing (n=36 each). The control group received Etomidate under anesthesia induction. The treatment group received dexmedetomidine and etomidate. Results There were no statistical significance in the changes of systolic and diastolic pressure at different time points between the treatment group and control, whereas the heart rate in both groups showed a significantly decreased trend ( P<0.05). The heart rate at medication and extubation in the treatment group was better than that in the control group was better than that in the control group (P<0.05). There were no statistically significant differences in the spontaneous breathing recovery time and extubation time between the two groups (P<0.05). The occurrence of adverse reactions, including postoperative agitation, nausea and vomiting, chill and respiratory depression in the treatment group were significant-ly less than those in the control group (P<0.05). Conclusion Using dexmedetomidine and etomidate can maintain stable heart rate in perioperative period, alleviate the occurrence of postoperative pain and infection, and decrease the occurrence of adverse reactions in recovery period, which has good clinical value.
