目的：探讨小切口复位联合外固定架与切开复位内固定术治疗老年桡骨远端骨折的临床效果。方法选取内蒙古医科大学附属医院骨科2010年4月~2014年4月收治的老年桡骨远端骨折患者70例，采用随机数表法分为对照组和试验组，每组各35例，其中对照组患者采用切开复位内固定术式治疗，试验组患者采用小切口复位联合外固定架术式治疗，比较两组患者骨折愈合时间，术后6、12个月腕关节功能评分优良率、桡骨远端掌倾角及尺偏角。结果试验组患者骨折愈合时间显著短于对照组，差异有统计学意义(P<0.05)；试验组患者术后6个月腕关节功能评分优良率高于对照组，桡骨远端掌倾角及尺偏角均大于对照组，差异有统计学意义(P<0.05)；两组患者术后12个月腕关节功能评分优良率、桡骨远端掌倾角及尺偏角比较，差异无统计学意义(P>0.05)。结论与切开复位内固定术式相比，小切口复位联合外固定架术式治疗老年桡骨远端骨折可有效促进骨折愈合和近期腕关节功能恢复，具有临床应用价值。
목적：탐토소절구복위연합외고정가여절개복위내고정술치료노년뇨골원단골절적림상효과。방법선취내몽고의과대학부속의원골과2010년4월~2014년4월수치적노년뇨골원단골절환자70례，채용수궤수표법분위대조조화시험조，매조각35례，기중대조조환자채용절개복위내고정술식치료，시험조환자채용소절구복위연합외고정가술식치료，비교량조환자골절유합시간，술후6、12개월완관절공능평분우량솔、뇨골원단장경각급척편각。결과시험조환자골절유합시간현저단우대조조，차이유통계학의의(P<0.05)；시험조환자술후6개월완관절공능평분우량솔고우대조조，뇨골원단장경각급척편각균대우대조조，차이유통계학의의(P<0.05)；량조환자술후12개월완관절공능평분우량솔、뇨골원단장경각급척편각비교，차이무통계학의의(P>0.05)。결론여절개복위내고정술식상비，소절구복위연합외고정가술식치료노년뇨골원단골절가유효촉진골절유합화근기완관절공능회복，구유림상응용개치。
Objective To investigate clinical efficacy of small incision reduction with external fixation and open reduc-tion and internal fixation in the treatment of elderly patients with distal radius fractures. Methods 70 elderly patients with distal radius fractures treated in Orthopedics Department of Affiliated Hospital of Inner Mongolia Medical Univer-sity from April 2010 to April 2014 were selected and divided into control group and experimental group by using a random number table method, with 35 cases in each group. Patients in control group were treated with open reduction and internal fixation, while patients in experimental group were treated with small incision reduction with external fixa-tion operation. The fracture healing time, the excellent and good rate of wrist joint function score, palmar tilt and ulnar deviation of distal radius in 6 and 12 months after operation between two groups were compared. Results The fracture healing time of experimental group was significantly shorter than that of control group, with statistical difference (P<0.05). The excellent and good rate of wrist joint function score in 6 months after operation in experimental group was higher than that in control group, palmar tilt and ulnar deviation of distal radius in 6 months after operation in experi-mental group were larger than those in control group, with statistical differences (P< 0.05). There were no statistical differences in excellent and good rate of wrist joint function score, palmar tilt and ulnar deviation of distal radius in 12 months after operation between two groups (P> 0.05). Conclusion Compared with open reduction and internal fixation, small incision reduction with external fixation in the treatment of elderly patients with distal radius fractures can effi-ciently promote fracture healing process and wrist joint function recovery in early period, which pos-sesses clinical application value.
