目的：观察胃镜检查前咽部麻醉不同给药时间、方法及镜检开始时间对受检者镜检不适程度的影响，以探寻最佳给药时间、给药方法及镜检开始时间。方法胃镜检查者891例按镜检顺序依次分列为A1-4、B1-4、C1-4三大组十二小组，分别于10秒钟内（A组）、5分钟（B组）、10分钟（C组）左右含服完利多卡因胶浆，并分别于含服完胶浆后即可镜检（A1、B1、C1组）、5分钟（A2、B2、C2组）、10分钟（A3、B3、C3组）、15分钟(A4、B4、C4组)左右开始镜检，观察各小组胃镜插入难易程度及耐受性，给予等级评分，借以评价受检者的不适程度。结果各组胃镜插入难易程度与受检者耐受性分值高低分布基本一致。（1）A、B、C个各大组内小组之间比较，均以含服完利多卡因胶浆后5分钟开始镜检组（即A2、B2、C2组）分值最低，即不适程度最低，与组内其他小组比较，均有显著性差异（p＜0.05）。（2）各个大组内分值最低之小组（A2、B2、C2组）间比较又以B2组为最低，与其余两组比较，差异有显著性（p＜0.05）。结论5分钟左右含服完利多卡因胶浆，并于含服完毕后5分钟左右开始胃镜检查，受检者不适程度最轻。
목적：관찰위경검사전인부마취불동급약시간、방법급경검개시시간대수검자경검불괄정도적영향，이탐심최가급약시간、급약방법급경검개시시간。방법위경검사자891례안경검순서의차분렬위A1-4、B1-4、C1-4삼대조십이소조，분별우10초종내（A조）、5분종（B조）、10분종（C조）좌우함복완리다잡인효장，병분별우함복완효장후즉가경검（A1、B1、C1조）、5분종（A2、B2、C2조）、10분종（A3、B3、C3조）、15분종(A4、B4、C4조)좌우개시경검，관찰각소조위경삽입난역정도급내수성，급여등급평분，차이평개수검자적불괄정도。결과각조위경삽입난역정도여수검자내수성분치고저분포기본일치。（1）A、B、C개각대조내소조지간비교，균이함복완리다잡인효장후5분종개시경검조（즉A2、B2、C2조）분치최저，즉불괄정도최저，여조내기타소조비교，균유현저성차이（p＜0.05）。（2）각개대조내분치최저지소조（A2、B2、C2조）간비교우이B2조위최저，여기여량조비교，차이유현저성（p＜0.05）。결론5분종좌우함복완리다잡인효장，병우함복완필후5분종좌우개시위경검사，수검자불괄정도최경。
Objective To observe effect of different throat anesthesia time, method and start time of examination on discomfort degree to patients before gastroscopy to ifnd the best anesthesia time, method and starting time of examination. Method divide 891 cases for gastroscopy test into three groups of A1-4、B1-4、C1-4 and 12 subroups according to sequence, and take lidocaine latex rubber in about 10 seconds (groupA), 5 minutes (group B), 10 minutes (Group C) respectively, and carry on gastroscopy after taking lidocaine right away(Group A1、B1、C1)and after about 5 minutes (Group A2、B2、C2), 10 minutes(Group A3、B3、C3)and 15 minutes (Group A4、B4、C4), (Group A4、B4、C4). Observe each group with gastroscope insertion dififcult degree and tolerance, and evaluate scores for discomfortness of patiens.Result distribution of gastroscope insertion dififcult degree of and tolerance scores of patients in each group was consistent. (1) compare subgroups of group A, B and C, scores in group of carrying on gastroscopy 5 minutes after taking lidocaine (which are group A2, B2, C2) are the lowest, which means the lightest discomfortness, and there is signiifcant difference (P < 0.05). (2) in subgroups with lowest scores among groups(Group A2、B2、C2), group B2 has the lowest score compared with the other two subgroups, and there is signiifcant difference (P < 0.05).Conclusion carrying on gastroscopy in 5 minutes after taking lidocaine during 5 minutes has lightest discomfortness for patients.
