目的：探讨多层螺旋CT血管造影（ MSCTA）在肝硬化血管改变及侧支循环中的临床应用价值。方法对138例肝硬化患者（肝硬化组）及50例无肝硬化的患者（对照组）行上腹部三期增强扫描，应用最大密度投影（ MIP）、多平面重组（ MPR）及容积再现（ VR）对肝动脉、门静脉及其分支进行血管重建成像，对获取的图像进行对比分析。结果两组的肝动脉、门静脉分支显示差异有统计学意义（ P＜0.01）；肝功能Child分级与门静脉的血管分级差异无统计学意义（ P＞0.05）。门静脉血管成像能清晰显示、了解侧支循环情况。27例脾功能亢进，23例肝癌患者介入治疗前通过血管成像进行血管评估。结论患者门静脉及肝动脉血管的改变可判断肝硬化程度，为临床治疗前评估血管改变提供可靠的影像依据。

Objective To evaluate the diagnostic value of multipie slice spiral CTA of hepatic vessels in the patients with liver cirrhosis.Methods Tri-phase enhanced CT scans of whole hepatic region were performed respec-tively in 138 patients with liver cirrhosis( the liver cirrhosis group) and in 50 patients without liver cirrhosis( the con-trol group) .The images of hepatic arteries, splenic arteriea and its branches were post-processed with MIP, MPR and VR, and their images were compared between the two groups.Results There were significant differences be-tween the two groups in showing hepatic artery,portal vein and its branches(P<0.01).There were no statistical differences in showing portal vein and Child grade system( P>0.05) .CTPV images displayed the portal venous sys-tem and its collateral circulation stereoscopically.The vascular assessments were performed in 27 patients with hyper-splenism and in 23 patients with liver cancer using CTA before interventional therapy.Conclusion The changes of hepatic arteries and splenic arteriea may indicate the severity of hepatic cirrhosis, and provide the reliable evidences for imaging diagnosis of the disease.
