目的：探讨白蛋白紫杉醇二线治疗一线含吉西他滨方案化疗失败的晚期胰腺癌患者的疗效和安全性。方法：6例一线含吉西他滨方案化疗失败的晚期胰腺癌患者，给予白蛋白紫杉醇100mg/ m2静脉滴注，d1、d8、d15，28天为1周期。治疗至疾病进展或出现不可耐受的不良反应。根据实体瘤疗效反应评价标准（RE-CIST）评价疗效，常规毒性判定标准（NCI - CTC 3.0）评价不良反应。结果：6例患者中位治疗时间为2周期（1～8周期），无 CR 病例，PR 1例，SD 3例，PD 2例。总有效率为16.7％，疾病控制率为66.7％，中位无疾病进展时间（PFS）为5.3个月，中位总生存时间（OS）为7个月，有2例患者仍存活，随访至今，生存时间分别为54个月和58个月。不良反应主要为骨髓抑制和神经毒性，多为Ⅰ-Ⅱ级，其中1例患者出现Ⅲ级白细胞下降，4例Ⅱ级下降；2例出现Ⅱ级肌肉酸痛，无Ⅳ级不良反应。结论：对于一线含吉西他滨方案化疗失败的晚期胰腺癌患者，白蛋白紫杉醇单药方案化疗可取得一定疗效，患者耐受性好，值得扩大样本量进一步研究。
목적：탐토백단백자삼순이선치료일선함길서타빈방안화료실패적만기이선암환자적료효화안전성。방법：6례일선함길서타빈방안화료실패적만기이선암환자，급여백단백자삼순100mg/ m2정맥적주，d1、d8、d15，28천위1주기。치료지질병진전혹출현불가내수적불량반응。근거실체류료효반응평개표준（RE-CIST）평개료효，상규독성판정표준（NCI - CTC 3.0）평개불량반응。결과：6례환자중위치료시간위2주기（1～8주기），무 CR 병례，PR 1례，SD 3례，PD 2례。총유효솔위16.7％，질병공제솔위66.7％，중위무질병진전시간（PFS）위5.3개월，중위총생존시간（OS）위7개월，유2례환자잉존활，수방지금，생존시간분별위54개월화58개월。불량반응주요위골수억제화신경독성，다위Ⅰ-Ⅱ급，기중1례환자출현Ⅲ급백세포하강，4례Ⅱ급하강；2례출현Ⅱ급기육산통，무Ⅳ급불량반응。결론：대우일선함길서타빈방안화료실패적만기이선암환자，백단백자삼순단약방안화료가취득일정료효，환자내수성호，치득확대양본량진일보연구。
Objective:To explore efficacy and safety of albumin paclitaxel second - line therapy with gemcitabine chemotherapy failure patients with advanced pancreatic cancer. Methods:Collect 6 cases with gemcitabine chemother-apy failure patients with advanced pancreatic cancer,were given Albumin paclitaxel 100mg/ m2 intravenous drip,d1 , d8 ,d15 ,28 days as a cycle. Repeat treatment until disease progression or not tolerant toxicity reaction. Results:All 6 patients were all able to evaluate curative effect. No CR,1 case PR,SD 3 cases,2 cases of PD. The total effective rate was 16. 7% ,the disease control rate 66. 7% ,the median time without disease progression(PFS)5. 3 months,the me-dian overall survival time 7 months. 2 patients were still alive,the survival time 54 months and 58 months respective-ly. Adverse reactions in 1 case of leukopenia and neutrophils III degree decline,4 cases with leukopenia and neutro-phils II degree,2 patients appeared II degree of muscle soreness. Conclusion:The first - line with gemcitabine chemo-therapy failure patients with advanced pancreatic cancer,Albumin paclitaxel palliative chemotherapy can obtain cer-tain curative effect,and safe.
