目的：观察手术切除术治疗胰腺头部实性假乳头状瘤的临床疗效。方法：回顾性分析2008年11月至2014年7月间，盛京医院普通外科14例因患胰腺头部实性假乳头状瘤而行手术治疗的患者临床资料，女性12例，男性2例，平均年龄36.2岁。8例行胰十二指肠切除术，2例行保留幽门的胰十二指肠切除术，2例行胰腺部分切除术，1例行胰十二指肠切除术联合门静脉切除术，1例行胰腺部分切除术联合胰体尾空肠Roux - en - Y 吻合术。评价手术效果，分析影像特点，并随访远期效果。结果：肿瘤平均7.1cm ×6.1cm，所有患者术后病理及免疫组化证实胰腺实性假乳头状瘤，平均住院时间26天，有5例发生术后胰瘘，发病率35.7％，无死亡病例。术后随访8～82个月，中位随访时间27.5个月，有1例于术后66个月复发，其余13例均无复发。结论：手术治疗胰腺头部实性假乳头状瘤是安全的，完整切除肿物远期疗效确切。
목적：관찰수술절제술치료이선두부실성가유두상류적림상료효。방법：회고성분석2008년11월지2014년7월간，성경의원보통외과14례인환이선두부실성가유두상류이행수술치료적환자림상자료，녀성12례，남성2례，평균년령36.2세。8례행이십이지장절제술，2례행보류유문적이십이지장절제술，2례행이선부분절제술，1례행이십이지장절제술연합문정맥절제술，1례행이선부분절제술연합이체미공장Roux - en - Y 문합술。평개수술효과，분석영상특점，병수방원기효과。결과：종류평균7.1cm ×6.1cm，소유환자술후병리급면역조화증실이선실성가유두상류，평균주원시간26천，유5례발생술후이루，발병솔35.7％，무사망병례。술후수방8～82개월，중위수방시간27.5개월，유1례우술후66개월복발，기여13례균무복발。결론：수술치료이선두부실성가유두상류시안전적，완정절제종물원기료효학절。
Objective:To observe the clinical effect of treating solid pseudopapillary neoplasms in the head of pan-creas by surgical resection. Methods:Retrospectively analyse the clinical data of 14 cases of solid pseudopapillary ne-oplasms in the head of pancreas treated by surgical resection from November 2008 to July 2014 at Shengjing Hospital. Female cases were 12 and male cases were 2,mean age was 36. 2 years old. 8 cases undergone pancreatoduodenecto-my,2cases pylorus preserved pancreatoduodenectomy,2 cases partial pancreatectomy,1 case pancreatoduodenectomy plus portal vein resection,1 case partial pancreatectomy plus pancreaticojejunostomy. The imaging features and cura-tive effect were evalued. Results:Of all the 14 tumors,the mean size was 7. 1cm × 6. 1cm. All the tumors were solid pseudopapillary neoplasms confirmed by pathology. The mean length of stay was 26 days and the rate of postoperation pancreatic fistula was 35. 7%(5 / 14). There were no patients died. After 8 to 82 months followed up,all the patients were good except one recurred 66 months after operation. Conclusion:Surgical treatment of solid pseudopapillary neo-plasms in the head of pancreas is safe. The long effect is excellent after complete resection of the tumor.
