目的:探讨分析肾透明细胞癌(CCRCC)的超声造影增强特点,探讨超声造影(CEUS)对于肾透明细胞癌定性诊断的临床价值。方法:回顾性分析经病理证实的78例 CCRCC 的 CEUS 图像,观察肿瘤的 CEUS增强模式、增强的均匀性以及肿瘤周边环状高增强(PHR)特征,并通过时间-强度曲线,定量分析相关参数。结果:CCRCC 相对正常肾皮质 CEUS 增强模式大多表现为快进(65/78,83.3%),快出(58/78,74.4%)和慢出(17/78,21.8%)。CEUS 下均匀性增强23例,非均匀性增强55例。PHR 的检出率为69.23%(54/78)。采用PHR 诊断肾透明细胞癌,其敏感性、特异性、准确性分别为69.23%、100%、60.3%。结论:超声造影在一定程度上揭示了 CCRCC 的生物学特征,CEUS 可为临床提供更多有价值的信息,肿瘤周边环状高增强(PHR)征象可作为诊断肾癌的特异性参考指标。
目的:探討分析腎透明細胞癌(CCRCC)的超聲造影增彊特點,探討超聲造影(CEUS)對于腎透明細胞癌定性診斷的臨床價值。方法:迴顧性分析經病理證實的78例 CCRCC 的 CEUS 圖像,觀察腫瘤的 CEUS增彊模式、增彊的均勻性以及腫瘤週邊環狀高增彊(PHR)特徵,併通過時間-彊度麯線,定量分析相關參數。結果:CCRCC 相對正常腎皮質 CEUS 增彊模式大多錶現為快進(65/78,83.3%),快齣(58/78,74.4%)和慢齣(17/78,21.8%)。CEUS 下均勻性增彊23例,非均勻性增彊55例。PHR 的檢齣率為69.23%(54/78)。採用PHR 診斷腎透明細胞癌,其敏感性、特異性、準確性分彆為69.23%、100%、60.3%。結論:超聲造影在一定程度上揭示瞭 CCRCC 的生物學特徵,CEUS 可為臨床提供更多有價值的信息,腫瘤週邊環狀高增彊(PHR)徵象可作為診斷腎癌的特異性參攷指標。

Objective:To discuss the characteristics of contrast - enhanced ultrasound(CEUS)images of renal clear cell carcinoma(CCRCC),and explore the clinical value of CEUS in qualitative diagnosis of CCRCC. Methods:We retrospectively analyzed 78 cases of CCRCC patientsˊ CEUS images,which were confirmed by pathology. We ob-served the tumor characteristics,such as tumor enhancement pattern,enhancement uniformity and the peritumoral hy-perenhanced rim(PHR)surrounding the tumor,and analyzed the related parameters quantitatively through time - in-tensity curves. Results:The enhanced model of CCECC mostly characterized by fast forward(83. 33% ),fast exit (74. 36% )and slow exit(21. 8% )relative to normal renal cortex. 23 cases showed homogeneous enhancement and 55 cases presented heterogeneity enhancement. PHR detection rate was 69. 23%(54 / 78). Using PHR diagnosis of re-nal clear cell carcinoma,the sensitivity,specificity and accuracy were 69. 23% ,100% and 60. 26% respectively. Conclusion:Contrast - enhanced ultrasound reveals the biological characteristics of CCRCC in a certain extent. CEUS can provide more valuable information for clinical. PHR index signs could be taken as reference to the specificity of the diagnosis of renal cancer.
