目的：研究直肠癌根治术后并发吻合口瘘的相关因素。方法：将2010年3月～2013年5月期间我院收治的240例行直肠前切除的直肠癌患者纳入研究，观察吻合口瘘的发生情况，并对危险因素进行调查和分析。结果：（1）共发生吻合口瘘13例，发生率5.42%；（2）不同性别、年龄、术前血清白蛋白水平、合并糖尿病情况、肿瘤分期、肿瘤分化程度、肿瘤位置、肿瘤距肛缘距离的患者，并发吻合口瘘的差异有统计学意义；（3）直肠癌根治术后吻合口瘘的发生情况与性别、年龄、合并糖尿病情况、肿瘤分期、肿瘤位置呈正相关，与术前血清白蛋白水平、肿瘤分化程度、肿瘤距肛缘距离呈负相关。结论：对于男性、高龄、低蛋白血症、合并糖尿病的低位直肠癌患者，需更严格地把握保肛手术指征，以预防术后吻合口瘘的发生。
목적：연구직장암근치술후병발문합구루적상관인소。방법：장2010년3월～2013년5월기간아원수치적240례행직장전절제적직장암환자납입연구，관찰문합구루적발생정황，병대위험인소진행조사화분석。결과：（1）공발생문합구루13례，발생솔5.42%；（2）불동성별、년령、술전혈청백단백수평、합병당뇨병정황、종류분기、종류분화정도、종류위치、종류거항연거리적환자，병발문합구루적차이유통계학의의；（3）직장암근치술후문합구루적발생정황여성별、년령、합병당뇨병정황、종류분기、종류위치정정상관，여술전혈청백단백수평、종류분화정도、종류거항연거리정부상관。결론：대우남성、고령、저단백혈증、합병당뇨병적저위직장암환자，수경엄격지파악보항수술지정，이예방술후문합구루적발생。
Objective: To study the related factors of anastomotic leakage after radical resection of rectal carcinoma.Methods:240 cases of rectal carcinoma patients in our hospital from March 2010 to May 2013 were enrolled and given anterior resection. The anastomotic leakage were observed and risk factors were investigated and analyzed.Results:(1)There were 13 cases of anastomotic leakage,the incidence was 5.42%; (2) there were statistical significance of difference in anastomotic leakage occurrence between patients with different sex, age, preoperative serum albumin level, diabetes, tumor stage, tumor differentiation, tumor location, tumor distance from the anal verge; (3) the occurrence of anastomotic leakage after radical resection were positively correlated with age, diabetes, tumor stage, tumor location and negatively correlated with preoperative serum albumin level, tumor differentiation, tumor distance from the anal verge.Conclusion:Indication of anus preserving operation should be strict grasped in male, age, hypoproteinemia, diabetic patients with low rectal cancer, so as to prevent the occurrence of postoperative anastomotic leakage.
