目的:应用二维斑点追踪成像技术观察射血分数正常的舒张性心力衰竭患者左室内外层心肌旋转特点,评价DHF患者内外层心肌收缩功能。方法临床确诊的舒张性心力衰竭(DHF)患者及正常对照组各32例。存储受检者超声心动图二维灰阶图像。使用Qlab7.0工作站进行脱机分析,记录左室短轴心尖、乳头肌及基底水平旋转角度峰值以及各水平内、外层心肌旋转角度。结果 DHF患者组左室整体扭转、各水平旋转及同水平内外层心肌旋转趋势与正常对照组一致。同一水平内层心肌旋转角度均大于外层心肌旋转角度。DHF患者组心尖水平内层心肌旋转角度较正常组减低。结论左室内层心肌旋转角度大于外层心肌旋转角度。 DHF患者左室心尖水平内层心肌旋转运动减弱,提示射血分数正常的DHF患者左室心肌收缩功能存在异常。

Objective To observe the characteristics of left ventricular (LV) endocardial and epidcardial rotation by two-dimensional speckle tracking imaging (2D-STI), and to evaluate the dif erent rotation in diastolic heart failure patients (DHF). Methods Sixty-four consecutive clinical y stable patients were enrol ed in this study. High frame rate dynamic two-dimensional images were recorded at the left ventricular short-axis view. Endocardial and epidcardial rotation angle were measured using Q-lab7.0 software of line. Results In al the subjects, the rotation of the endocardium was obviously greater than that of epicardium. In the apical plane, endocardial rotation was signi?cantly lower in heart failure groups than that in controls. Conclusion The rotation of the endocardium was obviously greater than that of epicardium. endocardial rotation was signi?cantly lower in DHF patients. Although LV ejection fraction in DHF patients was normal, the systolic function is impaired in DHF patients.
