目的探讨微波治疗仪联合纳米银对治疗高危型人乳头瘤病毒(HR-HPV)感染的疗效及护理措施,为临床治疗提供指导.方法选择选用2011年3月-2014年4月接受救治的197例合并不同程度宫颈病变且高危型人乳头瘤病毒(HR-HPV)呈阳性的患者作为研究对象.按照随机数字表法将研究对象分为对照组和试验组,对照组98例(49.75％)单独使用纳米银治疗,试验组99例(50.25％)使用微波治疗仪联合纳米银治疗.7d为一个疗程,治疗两个疗程采用心理、治疗、健康教育护理.治疗后复查HPV的转阴状况、IFN-γ 、IL-10的表达以及有无并发症状况.结果治疗后对照组患者HPV转阴率为57.14％ (56/98),试验组患者HPV转阴率为89.90％(89/99),存在统计学差异(P＜0.01);在宫颈组织IFN-γ表达方面,对照组和试验组治疗前后均存在显著差异(t=4.682、7.564,P＜0.01),治疗后试验组和对照组间存在差异(=2.455,P＜0.05);在IL-10表达状况方面,对照组和试验组治疗前后均存在显著差异(t=4.580、6.869,P＜0.01),治疗后试验组和对照组间存在差异(=2.463,P ＜0.05);两组患者均无严重的肝肾功能损伤.结论相对于单独使用纳米银,微波治疗仪联合纳米银治疗对合并宫颈病变的高危型人乳头瘤病毒感染更为有效、且并发症较少,值得临床推广.另外,采取相对应的护理工作是也是使治疗能顺利发展的关键点.

Objective To investigate the curative effect and nursing of microwave treatment instrument combined with silver nanoparticles on the treatment of high-risk human papillomavirus (HR-HPV) infection, which provide better guidance for clinical treatment.Methods 197 patients with different degree of cervical lesions and high-risk human papilloma virus (HR-HPV) positive patients were selected as the research objects from March 2011 to Apri 2014 and randomly were divided into research group and control group.98 cases in the control group (49.75％) were treated with nano silver alone.99 cases in research group (50.25％) were treated with microwave therapeutic instrument combined with nanometer silver.7 days was seted as a course and with 2 courses of treatment.All patients had no sex life, no a bath in tub in 2 months.Psychological nursing, treatment process nursing, and health education was used.HPV, IFN-γ,IL-10 and complications were analyzed after threatment.Results The HPV negative transformation rate of the control group was 57.14％ (56/98) and that of the study group was 89.90％ (89/99).There was significant difference between the two groups (P ＜ 0.01).For the expression of IFN-γ in cervical tissue, there were significant differences before and after treatment of the control group and the study group (t =4.682,7.564, P ＜ 0.01).There was significant difference between the two groups after treatment (t =2.455, P ＜0.05).For the expression of IL-10 in cervical tissue, there were significant differences before and after treatment of the control group and the study group (t =4.580, 6.869, P ＜0.01)).There was significant difference between the two groups after treatment (t =2.463, P ＜0.05).There were no serious damage to the liver and kidney function of the patients in two groups.Conclusions Microwave treatment combined with nanometer silver is more effective and less complications than only use of nanometer silver in treating cervical lesions infected with high-risk human papilloma virus.Corresponding nursing is also help for improving the conditions.
