目的:探讨 CT 值、磁共振氢质子波谱、磁共振三点式水脂分离(IDEAL)技术评估肝脏脂肪含量的价值。方法选取新疆医科大学第一附属医院动物中心同种成年 Beagle 犬15只。高脂饮食饲养后同期进行肝脏CT 平扫、磁共振氢质子波谱及 IDEAL 成像;测量肝脏同一部位 CT 值、1 H MRS 脂/水峰高度及峰下面积和IDEAL 脂/水信号比值;比较不同诊断方法的一致性。6只实验动物获得病理标本测量肝脏脂肪含量,评价3种影像方法与病理结果的相关性。结果在几种检测指标中,肝/脾 CT 比值与1 H MRS 脂/水峰高和峰下面积比值呈显著负相关(相关系数 r 分别为-0.74、-0.85,P <0.01),与病理结果对比,肝/脾 CT 比值、1 H MRS 脂/水峰高及峰下面积比值与病理结果相关性较高(r 分别为-0.943、0.886、0.943,P 值分别为0.005、0.019、0.005),而 IDEAL脂/水信号比值与病理结果无明显相关性(r =0.771,P =0.072)。结论CT、1 H MRS 与病理结果有较显著的相关性,为临床定量分析肝脂肪含量较可靠的方法;磁共振 IDEAL 技术与病理及其他影像检查的一致性较低。

목적:탐토 CT 치、자공진경질자파보、자공진삼점식수지분리(IDEAL)기술평고간장지방함량적개치。방법선취신강의과대학제일부속의원동물중심동충성년 Beagle 견15지。고지음식사양후동기진행간장CT 평소、자공진경질자파보급 IDEAL 성상;측량간장동일부위 CT 치、1 H MRS 지/수봉고도급봉하면적화IDEAL 지/수신호비치;비교불동진단방법적일치성。6지실험동물획득병리표본측량간장지방함량,평개3충영상방법여병리결과적상관성。결과재궤충검측지표중,간/비 CT 비치여1 H MRS 지/수봉고화봉하면적비치정현저부상관(상관계수 r 분별위-0.74、-0.85,P <0.01),여병리결과대비,간/비 CT 비치、1 H MRS 지/수봉고급봉하면적비치여병리결과상관성교고(r 분별위-0.943、0.886、0.943,P 치분별위0.005、0.019、0.005),이 IDEAL지/수신호비치여병리결과무명현상관성(r =0.771,P =0.072)。결론CT、1 H MRS 여병리결과유교현저적상관성,위림상정량분석간지방함량교가고적방법;자공진 IDEAL 기술여병리급기타영상검사적일치성교저。
Objective To evaluate the consistency of computed tomography (CT),1 H MR spectroscopy (1 H MRS)and Iterative decomposition of water and fat with echo asymmetry and least-squares estimation (IDEAL)in liver fat content detection.Methods 15 homogeneity high-fat die Beagle dogs were enrolled in the study,all which underwent plain CT scan,Single Voxel 1 H MRS analysis and IDEAL imaging syn-chronously.The variable indexes including the CT values,the 1 H MRS peaks of H 2 O and lipid,the areas under the peaks,the fat and water signal values of IDEAL were measured,and with correlation analysis.6 of these dogs also were conducted pathological contrast.Results All the above indexes,there are negative correlations existing between the CT value ratio and the areas under the 1 H-MRS peaks ratio (r =-0.743, P <0.01),also between CT value ratio and 1 H-MRS peaks ratio (r =-0.850,P <0.01).Compared with the pathologic results,the correlation coefficients were -0.943 (P =0.005),0.886 (P =0.019),0.943 (P=0.005)respectively.Conclusion There are significant relationships among CT,1 H-MRS and pathology and which are reliable methods for quantitative detecting of fatty liver.In this experiment,the IDEAL waterfat separate technique has no obvious correlation with pathology and other imaging techniques.
