目的通过meta分析全面、定量、系统地评价长春西汀在治疗脑梗死及后遗症中对神经功能损伤程度和生化指标的分析,为进一步指导临床治疗提供依据。方法检索国内相关数据库,查找长春西汀在急性脑梗死及后遗症中疗效的随机对照试验和临床对照试验,采用Stata软件进行Meta分析。结果26项研究描述了长春西汀对脑梗死神经缺损的治疗效果,发现长春西汀对神经功能恢复的效果优于对照药物;70项研究描述了长春西汀对脑梗死治疗的临床疗效,在对脑梗死治疗中,长春西汀优于其它对照药物,且联合用药有统计学意义;长春西汀对脑梗死患者血液流变学指标的影响如全血粘度、全血高切粘度、血浆粘度、红细胞聚集指数、纤维蛋白原、血小板聚集率、红细胞比积等均有显著改善作用。结论长春西汀能够有效改善急性脑梗死的脑部血液循环,最大限度降低病残率,对神经功能恢复有很好的疗效。

Objective Through systematical y assess the nerve function and biochemical index ef ect of vinpocetine in the treatment of acute and chronic cerebral infarction, to provide basis for further guidance on clinical treatment. Methods RCTs trials of vinpocetine in the treatment of acute and chronic cerebral infarction were searched in several database, meta-analyses were performed with Stata software. Results 26 studies showed that vinpocetine ef ects on neural functional recovery were bet er than that of the control drug;70 studies showed that ef ect of treatment of cerebral infarction is superior to control drug, and combining administration has more significance. Indexes of hemorheology in patients with cerebral infarction disease also had significant improved. Conclusion vinpocetine could improve the blood circulation on acute cerebral infarction patients, and maximum reduces movement disorders on Cerebral infarction sequel patients, have a good ef ect on the recovery of neural function, and is safety in clinic, but stil need more high quality randomized control ed trials to validate.
