目的探讨TEG普通杯图形呈直线的样本的肝素杯TEG与常规凝血试验的关系。方法23例患者TEG普通杯图形呈直线时,加做肝素酶中和试验进行肝素酶修正,同时做常规凝血试验和血常规检测。依据肝素酶中和试验的凝血指数(CI)分组,低凝组12例,正常组11例。采用线性相关和回归,比较各参数间的关系。结果①低凝组的R值、K值、PT、aPTT高于正常组,角度、MA、PLT低于正常组。②低凝组或正常组的R与K、角度、CI显著相关,角度与R、K、CI显著相关,CI与R、K、角度显著相关。③低凝组K、MA和PLT显著相关,K与R、角度、MA和CI相关,MA与K显著相关。④正常组R、K、角度、MA、CI均与PLT和凝血常规各指标不相关,K与R、角度和CI显著相关,MA与R、K、角度和CI均不相关。结论低凝组和正常组与常规凝血试验的关系不一致。 TEG普通杯图形呈直线时,应参考患者用药情况加做肝素酶中和试验,并与常规凝血试验结合,判断患者的凝血状态。

목적탐토TEG보통배도형정직선적양본적간소배TEG여상규응혈시험적관계。방법23례환자TEG보통배도형정직선시,가주간소매중화시험진행간소매수정,동시주상규응혈시험화혈상규검측。의거간소매중화시험적응혈지수(CI)분조,저응조12례,정상조11례。채용선성상관화회귀,비교각삼수간적관계。결과①저응조적R치、K치、PT、aPTT고우정상조,각도、MA、PLT저우정상조。②저응조혹정상조적R여K、각도、CI현저상관,각도여R、K、CI현저상관,CI여R、K、각도현저상관。③저응조K、MA화PLT현저상관,K여R、각도、MA화CI상관,MA여K현저상관。④정상조R、K、각도、MA、CI균여PLT화응혈상규각지표불상관,K여R、각도화CI현저상관,MA여R、K、각도화CI균불상관。결론저응조화정상조여상규응혈시험적관계불일치。 TEG보통배도형정직선시,응삼고환자용약정황가주간소매중화시험,병여상규응혈시험결합,판단환자적응혈상태。
Objective To investigate the relationship between heparinase cup TEG which plain cup showed a straight line and routine blood coagulation tests. Methods 23 patients were enrol ed in our study,after the trace of the plain cup showed straight line,a heparinase cup was used in order to neutralize heparin to repeat the test,the routine blood coagulation tests and the amount of PLT were measured at the same period. According to the CI value of heparinase cup TEG,23 patients were divided into two groups of A (12 cases with low coagulation status) and B (11 cases with normal coagulation status).linear correlation were car ied out to analyze relationship among the parameters.Results ①Compared to group B,the R time,K value,PT,aPTT were higher,while alpha angle,MA ,PLT were lower in group A. ②Either in group A or group B,there were significant cor elation between R time and K value, alpha angle or CI, alpha angle cor elated with R time,K value or CI, and CI cor elated with R time,K value or alpha angle significantly. ③In group A,significant cor elativity were observed between either K value or MA and PLT,the same results were obtained between R time and K value,alpha angle or CI, K value correlated with R time, alpha angle,MA or CI, MA correlated with R time significantly. ④There were no significantly cor elate between R time,K value, MA or CI and PLT or routine blood coagulation tests, significant cor elativity were observed between K value and R time,alpha angle or CI,there were no relationship between CI and R time,K value or alpha angle in group B.Conclusion There was a dif erent relationship between group A or B and routine blood coagulation tests.We should use a heparinase cup to repeat TEG test when the plain cup showed a straight line according to the clinical therapy.
