目的:探讨冠心病患者血小板补体受体表达的意义。方法:302例冠状动脉疾病纳入本次研究,其中包括稳定型冠状动脉疾病患者(n=152),不稳定型心绞痛患者(n=54),急性心肌梗塞患者(n=96),同期健康体检者276例纳入本次研究。流式细胞仪检测所有受试者血小板表面 C3aR 和 C5aR 表达水平。EILSA 检测所有受试者血清 C3a 和 C5a 含量。结果:稳定型和非稳定型冠状动脉疾病患者或急性心肌梗塞患者血小板表面 C5aR 表达水平明显高于健康对照者。而与健康对照者相比,稳定型和非稳定型冠状动脉疾病患者血小板表面 C3aR 表达水平显著升高,而急性心肌梗塞患者血小板表面 C3aR 表达水平未见明显变化。冠状动脉疾病患者血清 C3a 和 C5a 水平具有显著正相关性,而血小板表面 C3aR 和 C5aR 表达水平只与血清 C3a 含量呈弱相关性,与血清 C5a 含量无相关性。血小板表面 C3aR 和 C5aR 表达与血小板激活呈正相关(对于 C3aR,r=0.47,P>0.001,对于 C5aR,r=0.76, P<0.001)。同样地,C3aR 与血小板激活相关分子 SDF-1呈正相关。结论:冠状动脉疾病患者补体受体 C3aR 和C5aR 与血小板激活具有正相关性。

목적:탐토관심병환자혈소판보체수체표체적의의。방법:302례관상동맥질병납입본차연구,기중포괄은정형관상동맥질병환자(n=152),불은정형심교통환자(n=54),급성심기경새환자(n=96),동기건강체검자276례납입본차연구。류식세포의검측소유수시자혈소판표면 C3aR 화 C5aR 표체수평。EILSA 검측소유수시자혈청 C3a 화 C5a 함량。결과:은정형화비은정형관상동맥질병환자혹급성심기경새환자혈소판표면 C5aR 표체수평명현고우건강대조자。이여건강대조자상비,은정형화비은정형관상동맥질병환자혈소판표면 C3aR 표체수평현저승고,이급성심기경새환자혈소판표면 C3aR 표체수평미견명현변화。관상동맥질병환자혈청 C3a 화 C5a 수평구유현저정상관성,이혈소판표면 C3aR 화 C5aR 표체수평지여혈청 C3a 함량정약상관성,여혈청 C5a 함량무상관성。혈소판표면 C3aR 화 C5aR 표체여혈소판격활정정상관(대우 C3aR,r=0.47,P>0.001,대우 C5aR,r=0.76, P<0.001)。동양지,C3aR 여혈소판격활상관분자 SDF-1정정상관。결론:관상동맥질병환자보체수체 C3aR 화C5aR 여혈소판격활구유정상관성。
ObjectiveTo investigate the clinical significance of Expression of anaphylatoxin receptors C3a and C5a on platelets in patients with coronary heart disease. Methods Expression levels of C3aR and C5aR were measured by flow cytom-etry in a collective of 302 patients with documented coronary artery disease (CAD) including patients with stable CAD (n =152), unstable angina(n=54), acute myocardial infarction (n=96) or healthy controls (n=276). Results Patients with stable CAD, un-stable angina or AMI had significantly higher expression of C5aR on platelets in comparison to healthy controls. In contrast, the expression of C3aR on platelets was significantly enhanced in patients with stable and unstable CAD but not in patients with AMI compared to controls. While there was a strong correlation between the soluble ligands of these receptors C3a and C5a, we observed only a weak correlation with their receptors on platelets. Of note, the expression of both anaphylatoxin-receptors on platelets strongly correlated with platelet activation as assessed with the surface activation marker P-selectin. Likewise, we observed a positive correlation of C3aR with other molecules associated with platelet activation such as SDF-1. Conclusion we observed a positive correlation between the expression of anaphylatoxin-receptors C3aR and C5aR with platelet activation in patients with CAD.
