目的:分析阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrom,OSAHS)患儿血清胰岛素样生长因子(insulin-like growth factor-1, IGF-1)、胰岛素样生长因子结合蛋白-3(insulin-like growth factor-binding protein 3,IGFBP-3)水平的变化。方法随机选择3~6岁OSAHS患儿50例,选择10例健康儿童作为对照组,检测所有儿童血清的IGF-1和IGFBP-3水平,应用SPSS17进行统计学处理。结果重度组OSAHS患儿血清IGF-1水平与对照组比较有显著性差异(t=4.275,P<0.05),其余OSAHS组与对照组比较均无显著差异(P均>0.05)。重度组OSAHS患儿的血清IGF-1水平与呼吸暂停低通气指数(apnea hypopnea index,AHI)呈负相关(r=-0.505,P<0.05),中、重度组OSAHS患儿血清IGF-1水平与平均最低血氧饱和度(lowest oxygen saturation,LSaO2)呈正相关(r=0.61、0.553,P<0.05)。各组OSAHS患儿血清IGF-1水平与慢波睡眠(slow wave sleep,SWS)无显著相关(P均>0.05)。各组OSAHS患儿血清IGFBP-3水平与AHI、平均LSaO2、SWS睡眠均无显著相关(P均>0.05)。结论儿童血清IGF-1水平较血清IGFBP-3水平易受OSAHS的影响,OSAHS患儿生长发育迟缓可能与血清IGF-1水平降低有关。

목적:분석조새성수면호흡잠정저통기종합정(obstructive sleep apnea hypopnea syndrom,OSAHS)환인혈청이도소양생장인자(insulin-like growth factor-1, IGF-1)、이도소양생장인자결합단백-3(insulin-like growth factor-binding protein 3,IGFBP-3)수평적변화。방법수궤선택3~6세OSAHS환인50례,선택10례건강인동작위대조조,검측소유인동혈청적IGF-1화IGFBP-3수평,응용SPSS17진행통계학처리。결과중도조OSAHS환인혈청IGF-1수평여대조조비교유현저성차이(t=4.275,P<0.05),기여OSAHS조여대조조비교균무현저차이(P균>0.05)。중도조OSAHS환인적혈청IGF-1수평여호흡잠정저통기지수(apnea hypopnea index,AHI)정부상관(r=-0.505,P<0.05),중、중도조OSAHS환인혈청IGF-1수평여평균최저혈양포화도(lowest oxygen saturation,LSaO2)정정상관(r=0.61、0.553,P<0.05)。각조OSAHS환인혈청IGF-1수평여만파수면(slow wave sleep,SWS)무현저상관(P균>0.05)。각조OSAHS환인혈청IGFBP-3수평여AHI、평균LSaO2、SWS수면균무현저상관(P균>0.05)。결론인동혈청IGF-1수평교혈청IGFBP-3수평역수OSAHS적영향,OSAHS환인생장발육지완가능여혈청IGF-1수평강저유관。
[ABSTRACT]OBJECTIVETo analyse the levels of serum IGF-1、IGFBP-3 in children with obstructive sleep apnea hyponea syndrome (OSAHS).METHODS50 children with OSAHS and 10 healthy children were enrolled in this study. According to the respiratory disturbance index (AHI). Serum IGF-1、IGFBP-3 of children were determined and the results were analyze.RESULTSSerum level of IGF-1 had significant statistical differences between severe OSAHS group and controls (t=4.275,P<0.05). Serum level of IGFBP-3 had no significant statistical differences between OSAHS children and controls (P>0.05). Serum IGF-1 of severe OSAHS children was negatively correlated with AHI (r=-0.505,P<0.05). Serum level of IGF-1 in moderate and severe OSAHS children were positively correlated with lowest average blood oxygen saturation (r=0.61, 0.553,P<0.05). Serum level of IGF-1 in OSAHS children had no significant correlation with SWS sleep (P>0.05). Serum level of IGFBP-3 in OSAHS children had no significant correlation with AHI, lowest average blood oxygen saturation and SWS sleep (P>0.05).CONCLUSION Serum levels of IGF-1 in children are more easily affected by OSAHS than serum level of IGFBP-3. Growth retardation of children with OSAHS may be related to the decrease of IGF-1 concentration.
