目的总结射频消融术( RFA)治疗宫颈妊娠的经验. 方法 2005年6月至2013年11月,应用超声引导下RFA治疗宫颈妊娠5例. 结果 2例单纯行RFA治疗成功,术中出血少;2例行RFA联合宫颈局部注射MTX治疗成功,术中出血少;1例行RFA治疗时出血多,宫颈碘仿纱布填塞2 d取出后阴道活动性出血,行双侧子宫动脉栓塞+MTX双侧子宫动脉灌注,1 w后在超声引导下行宫颈妊娠残留物清除术. 5例术后12~21 d血β-hCG转阴,术后第2个月均有正常月经来潮. 组织病理报告:绒毛组织. 结论阴道出血少、血β-hCG值不高、妊娠囊小、无胎心搏动的宫颈妊娠,可单纯应用RFA治疗,是一种安全、经济的手术方法,具有微创、对出血部位直接电凝止血、出血少的特点,有很好的临床效果.

Objective The objective of this study was to explore the effectiveness of radio frequency ablation in the treatment of cervical pregnancy.Methods In June 2005 to November 2013, a total of 5 patients with cervical pregnancy were treated by ultrasound guided radiofrequency ablation.Results 2 cases treated with radiofrequency ablation alone succeeded, less intraoperative bleeding;2 cases treated with radiofrequency ablation combined cervical local injection of MTX successfully, less intraoperative bleeding; 1 case treated with radiofrequency ablation was bleeding more, padded cervical iodoform gauze out after 2 days.For vaginal active bleeding, bilateral uterine artery embolization combined MTX bilateral uterine artery perfusion.After one week in removal of cervical pregnancy residues guided by ultrasound.5 cases of postoperative 12 ~21 days blood beta HCG were normal, after 2 months with normal menstrual cramps. Histopathologic report: villi tissue. Conclusions With less bleeding and blood beta HCG value is not high, small gestational sac, no heart throb of the cervical pregnancy, can be simply used radiofrequency ablation.It is a safe and economic operation method, with minimally invasive, bleeding direct an electrocautery unit of blood, and the characteristics of less bleeding, has good clinical effect.
