目的：探讨第二产程延长的原因及延长后导致新生儿窒息的高危因素。方法：筛选2013年1月~2014年1月于我院进行分娩并发生第二产程延长的产妇106例作为研究对象。对所有产妇应用超声进行产前检查观察胎儿体位等情况，分娩过程前观察产妇产道、胎儿体位及胎儿娩出时脐带位置，分析产妇第二产程延长的原因及造成新生儿窒息的高危因素。结果：106例产妇均发生第二产程延长，其中造成第二产程延长的前四位原因为：胎儿体位异常37例（34．91％），产妇宫缩乏力15例（14．15％），脐带位置异常22（20．75％），产道异常14例（13．21％）。其中发生新生儿窒息14例。结论：造成第二产程延长的主要原因为胎儿体位异常、产妇宫缩乏力、产道异常、脐带位置异常，同时胎儿窘迫、脐带位置异常是导致第二产程延长后新生儿窒息的主要因素。

목적：탐토제이산정연장적원인급연장후도치신생인질식적고위인소。방법：사선2013년1월~2014년1월우아원진행분면병발생제이산정연장적산부106례작위연구대상。대소유산부응용초성진행산전검사관찰태인체위등정황，분면과정전관찰산부산도、태인체위급태인면출시제대위치，분석산부제이산정연장적원인급조성신생인질식적고위인소。결과：106례산부균발생제이산정연장，기중조성제이산정연장적전사위원인위：태인체위이상37례（34．91％），산부궁축핍력15례（14．15％），제대위치이상22（20．75％），산도이상14례（13．21％）。기중발생신생인질식14례。결론：조성제이산정연장적주요원인위태인체위이상、산부궁축핍력、산도이상、제대위치이상，동시태인군박、제대위치이상시도치제이산정연장후신생인질식적주요인소。
Objective:To investigate the risk factors leading to neonatal asphyxia investigate the cause of prolonged second stage of labor and after prolonged. Methods:Selected second prolonged maternal from January 2013 - January 2014 for the study. Of all maternal prenatal ultrasound examination to observe fe-tal position,etc. ,before the observation of maternal birth canal during delivery,when the umbilical cord fetal position and the position of the fetus,the mater-nal prolonged second stage of analysis of causes and risk factors caused by neonatal asphyxia. Results:106 cases of maternal occurred prolonged second stage of labor,which caused prolonged second stage of the top four reasons as follows:37 cases of abnormal fetal position(34. 91% ),maternal uterine inertia in 15 cases(14. 15% ),umbilical cord abnormal position 22(20. 75% ),abnormal birth canal 14 cases(13. 21% ). Where 14 cases of neonatal asphyxia. Conclusion:Resulting in prolonged second stage of the main reasons abnormal maternal uterine inertia,birth canal abnormal fetal position,umbilical cord ab-normal position,while fetal distress,cord abnormal position of the main factors leading to the second stage of labor prolonged neonatal asphyxia.
