目的 探讨瑞舒伐他汀对高血压病并血脂异常患者左室肥厚及血浆脑钠肽( BNP )水平的影响. 方法 选取高血压病并血脂异常患者120例,随机分为试验组及对照组,每组60例. 两组均予降压治疗,试验组加用瑞舒伐他汀10 mg/d,治疗时间为1年. 观察治疗前后血压、血脂、BNP、左室质量指数(LVMI)和相对室壁厚度(RWT)的变化. 结果 (1)试验组及对照组(所有研究对象)治疗前相关分析示BNP与LVMI、RWT分别呈正相关(P<0.05). (2)两组治疗1年后,与治疗前相比血压、BNP、LVMI、RWT水平均明显下降(P<0.05),与对照组相比,试验组BNP、LVMI、RWT水平下降更显著(P<0.05). (3)试验组治疗后总胆固醇( TC)、三酰甘油( TG)、低密度脂蛋白胆固醇( LDL-C)水平较治疗前明显下降( P<0.05),高密度脂蛋白胆固醇(HDL-C)水平明显升高(P<0.05),而对照组治疗前后比较血脂水平无明显变化(P>0.05),两组治疗后TC、LDL-C、HDL-C水平比较有显著差异(P<0.05). 结论 高血压病患者血浆BNP水平升高与左室肥厚有明显关系,瑞舒伐他汀具有改善高血压病患者左室肥厚及降低血浆BNP的作用.

목적 탐토서서벌타정대고혈압병병혈지이상환자좌실비후급혈장뇌납태( BNP )수평적영향. 방법 선취고혈압병병혈지이상환자120례,수궤분위시험조급대조조,매조60례. 량조균여강압치료,시험조가용서서벌타정10 mg/d,치료시간위1년. 관찰치료전후혈압、혈지、BNP、좌실질량지수(LVMI)화상대실벽후도(RWT)적변화. 결과 (1)시험조급대조조(소유연구대상)치료전상관분석시BNP여LVMI、RWT분별정정상관(P<0.05). (2)량조치료1년후,여치료전상비혈압、BNP、LVMI、RWT수평균명현하강(P<0.05),여대조조상비,시험조BNP、LVMI、RWT수평하강경현저(P<0.05). (3)시험조치료후총담고순( TC)、삼선감유( TG)、저밀도지단백담고순( LDL-C)수평교치료전명현하강( P<0.05),고밀도지단백담고순(HDL-C)수평명현승고(P<0.05),이대조조치료전후비교혈지수평무명현변화(P>0.05),량조치료후TC、LDL-C、HDL-C수평비교유현저차이(P<0.05). 결론 고혈압병환자혈장BNP수평승고여좌실비후유명현관계,서서벌타정구유개선고혈압병환자좌실비후급강저혈장BNP적작용.
Objective To study the effects of Rosuvastatin on left ventricular hypertrophy and the plasma BNP level in patients with hypertension and hyperlipidemia.Methods 120 patients with hypertension and hyperlipidemia were randomly divided into the treatment group and the control group( each n=60 ) .Both groups were treated antihypertensive treatment, and the treatment group was additionally treated with Rosuvastatin (10mg/d).The treatment lasted one year.The changes of blood pressure, blood lipids, BNP, left ventricular mass index( LVMI) and relative wall thickness( RWT) in all patients were measured before and after treatment. Results (1) Pearson linear correlation analysis in all patients before treatment showed that BNP was positively correlated with LVMI and RWT respectively(P<0.05).(2) Blood pressure, BNP, LVMI and RWT decreased significantly in two groups after one year of treatment(P<0.05).Compared with the control group, BNP, LVMI and RWT of the treatment group decreased more significantly(P<0.05).(3) Levels of total cholesterol (TC), triglyceride (TG) and low density lipoprotein-cholesterol(LDL-C) decreased significantly in the treatment group after treatment(P<0.05), Levels of HDL-C increased significantly(P<0.05).However, significant difference of blood lipids level was not found in control group (P>0.05) .The comparison of TC, LDL-C and HDL-C between two groups was significantly different after the treatment(P <0.05).Conclusions There is obvious association of elevating BNP with left ventricular hypertrophy in patients with hypertension.The treatment of Rosuvastatin in patients with hypertension can improve left ventricular hypertrophy and decrease the plasma BNP level.
