目的:探讨高尔基体蛋白73(GP73)、甲胎蛋白异质体3(AFP-L3)和磷脂酰肌醇蛋白聚糖-3(GPC-3)联合检测在原发性肝癌(PHC)诊断中的临床价值。方法采用免疫组织化学法和酶联免疫法检测原发性肝癌(PHC 组,n=34例)、肝硬化(LC 组,n=25例)和病毒性肝炎(VH 组,n=27例)患者肝组织和血清 GP73、AFP-L3和 GPC-3的表达水平,同期选择30例门诊正常体检者作为对照组。观察各组肝组织和血清 GP73、AFP-L3和 GPC-3的表达水平,并比较 GP73、AFP-L3和 GPC-3单一和联合检测 PHC 的阳性率。结果PHC 组肝组织及血清 AFP-L3、GP73和GPC-3表达水平均明显高于其他各组(P <0.05),3项标志物(AFP-L3、GP73和 GPC-3)单项检测 PHC 的阳性率分别为70.6%、76.5%、79.4%,两两指标联合检测 PHC 的阳性率可至82.3%、85.3%和88.2%,3项指标联合检测 PHC的阳性率可高达91.2%,明显高于单项检测率(P <0.05)。结论AFP-L3、GP73和 GPC-3联合检测可提高 PHC 阳性率,对早期 PHC 诊断具有重要的临床意义。

목적:탐토고이기체단백73(GP73)、갑태단백이질체3(AFP-L3)화린지선기순단백취당-3(GPC-3)연합검측재원발성간암(PHC)진단중적림상개치。방법채용면역조직화학법화매련면역법검측원발성간암(PHC 조,n=34례)、간경화(LC 조,n=25례)화병독성간염(VH 조,n=27례)환자간조직화혈청 GP73、AFP-L3화 GPC-3적표체수평,동기선택30례문진정상체검자작위대조조。관찰각조간조직화혈청 GP73、AFP-L3화 GPC-3적표체수평,병비교 GP73、AFP-L3화 GPC-3단일화연합검측 PHC 적양성솔。결과PHC 조간조직급혈청 AFP-L3、GP73화GPC-3표체수평균명현고우기타각조(P <0.05),3항표지물(AFP-L3、GP73화 GPC-3)단항검측 PHC 적양성솔분별위70.6%、76.5%、79.4%,량량지표연합검측 PHC 적양성솔가지82.3%、85.3%화88.2%,3항지표연합검측 PHC적양성솔가고체91.2%,명현고우단항검측솔(P <0.05)。결론AFP-L3、GP73화 GPC-3연합검측가제고 PHC 양성솔,대조기 PHC 진단구유중요적림상의의。
Objective To investigate the clinical value of Golgi protein 73 (GP73),alpha fetoprotein heterogeneity 3 (AFP-L3)combined Glypican-3(GPC-3)detection with in the diagnosis of primary hepatic carcinoma.Methods The expression levels of GP73,AFP-L3 and GPC-3 in liver tissue and serum of primary hepatic carcinoma (group PHC,n=34),liver cirrhosis (group LC,n=25)and viral hepatitis (group VH,n=27)patients were detected by immunohisto-chemical and enzyme immunoassay.At the same period,30 cases of normal examination were selected as control group. The expression levels of GP73,AFP-L3 and GPC-3 in liver tissue and serum of all groups were observed,and compared the positive rate of GP73,AFP-L3,GPC-3 single detection and combined detection to primary hepatic carcinoma.Results The expression levels of GP73,AFP-L3 and GPC-3 in the liver tissue and serum of PHC group were significantly higher than that of other groups (P <0.05).The single test positive rate of AFP-L3,GP73 and GPC-3 were 70.6%, 76.5%,79.4% to PHC,two indicators combined detection of PHC were 82.3%,85.3% and 88.2%,3 indicators com-bined detection of PHC was 91.2%.The rate of 3 indicators combined detection was higher than that of single indicator (P <0.05).Conclusion The combined detection of AFP-L3,GP73 and GPC-3 could improve the positive rate of PHC. It had an important clinical significance for early diagnosis of PHC.
