目的:探讨 PD 伴抑郁与脑组织感兴趣区微观结构变化的相关性。方法根据 DSM-IV 将40例 PD 患者分成 PD 伴抑郁组和 PD 非抑郁组;对2组进行 DTI 检查,测定感兴趣区的 FA 值,对 PD 伴抑郁组患者行汉密尔顿抑郁量表评估。结果PD 伴抑郁组额叶白质 FA 值(0.307±0.044)、扣带回前部白质 FA值(0.297±0.02)较 PD 非抑郁组额叶白质 FA 值(0.370±0.31)、扣带回前部白质 FA 值(0.324±0.02)明显下降,(P <0.05),其余各 ROI 的 FA 值无明显差异(P >0.05)。PD 伴抑郁与额叶白质及扣带回前部白质 FA值有关(OR=0.00,OR=0.00,P <0.05)。PD 伴抑郁组患者额叶白质、扣带回前部白质 FA 值与 HAMD 量表得分呈负相关(r=-0.615,r=-0.515,P <0.05)结论PD 伴抑郁患者额叶及扣带回前部白质微观结构存在变化,且此种变化的严重程度与抑郁程度有关。

목적:탐토 PD 반억욱여뇌조직감흥취구미관결구변화적상관성。방법근거 DSM-IV 장40례 PD 환자분성 PD 반억욱조화 PD 비억욱조;대2조진행 DTI 검사,측정감흥취구적 FA 치,대 PD 반억욱조환자행한밀이돈억욱량표평고。결과PD 반억욱조액협백질 FA 치(0.307±0.044)、구대회전부백질 FA치(0.297±0.02)교 PD 비억욱조액협백질 FA 치(0.370±0.31)、구대회전부백질 FA 치(0.324±0.02)명현하강,(P <0.05),기여각 ROI 적 FA 치무명현차이(P >0.05)。PD 반억욱여액협백질급구대회전부백질 FA치유관(OR=0.00,OR=0.00,P <0.05)。PD 반억욱조환자액협백질、구대회전부백질 FA 치여 HAMD 량표득분정부상관(r=-0.615,r=-0.515,P <0.05)결론PD 반억욱환자액협급구대회전부백질미관결구존재변화,차차충변화적엄중정도여억욱정도유관。
Objective To investigate the relation between depression in Parkinson’s Disease(PD)and the microstructure changes of brain regions of interest(ROIs)in diffusion tensor imaging(DTI).Methods De-pending on DSM-IV,40 PD patients were divided into DPD (Depression of Parkinson's disease)group and PDND (Parkinson's disease without depression )group.Both groups were measured by DTI to get the frac-tional anisotropy (FA)values of ROIs.DPD group were studied by Hamilton depression scale (HAMD)to as-sess the degree of depression.Results The PD group showed significant FA reduction in FWM,ACLB (0.307 ±0.044,0.297±0.02 VS 0.370±0.3 1 ,0.324±0.02;P <0.05).In the Logistic Regression analysis,FA in FWM and ACLB show significant correlation to DPD (OR = 0.00,OR = 0.00,P < 0.05 ).The scores of HAMD in DPD group was negatively correlated with the FA values of FWM and ACLB (r = -0.61 5,r =-0.5 1 5,P <0.05).Conclusions The microstructural damage in FWM and ACLB is positively correlated with cognitive impairment in PD patients.The FA reduction of FWN and ACLB has the potential to serve as a non-invasive early biomarker for DPD.
