目的：探讨超声内镜联合MRI扫描在胃癌术前分期中的临床价值。方法：选取2014年5月-2015年8月经手术组织病理证实的胃癌患者31例，术前均行超声内镜与MRI联合检查并判定TN分期，以组织病理TN分期为金标准，比较术前超声内镜与MRI联合判定的TN分期与金标准的一致性。结果：与组织病理TN分期结果比较，超声内镜联合MRI判定T、N分期的符合率均为80.6%，两者的结果具有一致性（Kappa=0.65， P<0.05）。结论：术前联合应用超声内镜、MRI可以显著提高胃癌术前TN分期的准确性，具有重要的临床价值。
목적：탐토초성내경연합MRI소묘재위암술전분기중적림상개치。방법：선취2014년5월-2015년8월경수술조직병리증실적위암환자31례，술전균행초성내경여MRI연합검사병판정TN분기，이조직병리TN분기위금표준，비교술전초성내경여MRI연합판정적TN분기여금표준적일치성。결과：여조직병리TN분기결과비교，초성내경연합MRI판정T、N분기적부합솔균위80.6%，량자적결과구유일치성（Kappa=0.65， P<0.05）。결론：술전연합응용초성내경、MRI가이현저제고위암술전TN분기적준학성，구유중요적림상개치。
Objective:To explore the clinical value of preoperative endoscopic ultrasound(EUS)combined with MRI for prediction of TN stage of gastric cancer before operation.Method:31 case of pathologically confirmed gastric carcinoma were selected from May 2014 to August 2015,they received preoperative endoscopic ultrasound (EUS)combined with MRI for prediction of TN stage.The pathological TN stage was regarded as the gold standard.The coherence of preoperative signs on combined EUS and MRI scan was compared with the gold standard.Result:Compared with histopathologic results, the coherence rates of T and N stage in EUS combined with MRI scan all were 80.6%,the result had a good coherence (Kappa=0.65,P<0.05).Conclusion:As a conclusion,combined EUS and MRI has a high predictive value for TN stage of gastric cancer before operation.
