目的 研究老年人热休克蛋白60(HSP60)、人热休克蛋白60 IgG(HSP60IgG)和牙龈卟啉单胞菌(P.g)HSP60IgG与冠心病和牙周炎间的关系. 方法 回顾性分析2012年9-10月天津医科大学第二医院老干部查体患者128例,分为4组,即健康对照组30例、冠心病组27例、牙周炎组36例、牙周炎并存冠心病(合并组)35例,用酶联免疫吸附法(ELISA)测定4组人HSP60、人HSP60IgG、P.gHSP60IgG的浓度,同时完善一般生化指标如肝肾功能、血脂等的检查. 结果 冠心病组、牙周炎组、合并组人HSP60、人HSP60IgG、P.gHSP60IgG浓度高于健康对照组(均P<0.05);合并组上述指标亦高于冠心病组和牙周炎组,牙周炎组P.gHSP60IgG浓度高于冠心病组(P<0.05).经偏相关分析结果显示,人HSP60和人HSP60IgG呈正相关(r=0.705,P<0.01),P.gHSP60IgG和人HSP60IgG呈正相关(r=0.825,P<0.01).经多元Logistic回归分析人HSP60、人HSP60IgG和P.gHSP60IgG可能是冠心病的危险因素(P<0.05). 结论 人HSP60及HSP60IgG、P.gHSP60 IgG可能在冠心病和牙周炎间起一定的桥接作用,患有牙周炎的老年人可能更易发生冠心病.

Objective To investigate the relationships of human heat shock protein 60 (HSP 60),heat shock protein 60 IgG (HSP60 IgG),heat shock protein 60 IgG from porphyromonous gingivalis (P.g HSP60 IgG) with coronary heart disease and periodontitis,in order to provide the theoretical basis on the new risk factors for coronary heart disease.Methods A total of 128 veteran cadres who took health physical examination in the Second Hospital of Tianjin Medical University from September to October in 2012 were recruited.They were divided into 4 groups:the healthy control group (n=30),coronary heart disease group (CHD group,n=27),the periodontitis group (n=36),CHD combined with periodontitis group (n =35).The concentrations of HSP60,HSP60 IgG,P.g HSP60 IgG were determined by enzyme linked immunosorbent assay (ELISA),and the general biochemical markers such as liver and kidney function,blood lipids were detected at the same time.Results The concentrations of human HSP60,HSP60 IgG,P.gHSP60 IgG were higher in CHD,periodontitis and CHD combined with periodontitis groups than in the healthy control group (all P<0.05).The above indicators in coronary heart disease combined with periodontitis group were also increased compared with those in CHD and periodontitis groups (all P<0.05).The patients with periodontitis had higher P.g HSP60 IgG concentration than patients with CHD (P<0.05).The partial correlation analysis showed that HSP60 and P.g Hsp60 IgG levels were positively correlated with human HSP60IgG level (r=0.705,0.825,P<0.01).Multivariate logistic regression analysis showed that human HSP60,HSP60IgG and P.gHsp60IgG may be the risk factors for coronary heart disease (all P<0.05).Conclusions Human HSP60,HSP60 IgG and P.gHSP60 IgG may play a bridging role between coronary heart disease and periodontitis.The elderly patients with periodontitis may be more susceptible to coronary heart disease.
