目的:分析我院门诊处方不合理的原因及存在的问题,为临床合理用药提供参考.方法:采用回顾性调查,随机抽取2013年1月~6月我院门诊处方共3 000张,对处方进行点评及对不合理处方进行分析.结果:不合理处方占调查处方的比例为9.83%,其中用法用量不适宜最多占3.3%,其次为未写诊断或诊断书写不全占1.6%.结论:用法用量不适宜与未写诊断或诊断书写不全为我院不合理用药的主要表现形式,创新合理用药举措尤为必要.

목적:분석아원문진처방불합리적원인급존재적문제,위림상합리용약제공삼고.방법:채용회고성조사,수궤추취2013년1월~6월아원문진처방공3 000장,대처방진행점평급대불합리처방진행분석.결과:불합리처방점조사처방적비례위9.83%,기중용법용량불괄의최다점3.3%,기차위미사진단혹진단서사불전점1.6%.결론:용법용량불괄의여미사진단혹진단서사불전위아원불합리용약적주요표현형식,창신합리용약거조우위필요.
Objective: To analyze the problems and reasons of the irrational outpatient prescriptions, and to provide reference for clinical rational drug use.Methods:A retrospective study was conducted on 3 000 randomly selected outpatient prescriptions in our hospital from January to June 2013, to comment on those prescriptions and then analyze the irrational ones of among them.Results:Unreasonable prescriptions accounted for the proportion of survey prescription 9.83%,which is not suitable dosage up to 3.3%, followed by the lack of diagnosis or incomplete diagnosis which accounted for 1.6%.Conclusion: Improper dosage, lack of diagnosis together with incomplete diagnosis were the main manifestations of the irrational drug use in our hospital, therefore, the innovation of the rational drug use is particularly necessary.
