目的:观察超声心动图应用于妊娠期糖尿病胎儿心功能中的评价效果.方法:选取我院2012年1月~2014年8月期间90例妊娠期糖尿病(GDM)孕妇,按照其血糖水平分组为甲、乙组各45例;以同期健康单胎妊娠孕妇45例为对照组.并采用超声心动图检测胎儿心脏肺动脉瓣环内径(PVA)、心脏二尖瓣环内径(MVA)、主动脉瓣环内径(AVA)、三尖瓣环内径(TVA)、左/右室舒张末期内径(LVDd、RVDd)、左/右室壁厚度(LVWT、RVWT)、左/右室收缩末期内径(LVDs、RVDs)等.结果:乙组胎儿TVA、LVMPI、MVA、RVMPI及PVA均明显大于对照组,P<0.05;甲组TVA、PVA、MVA、AVA及RVMPI均明显小于乙组,P<0.05.甲组各瓣环内径与对照组比较,P>0.05;而其LVMPI及RVMPI均明显大于对照组,P<0.05.结论:孕妇血糖水平越高,胎儿心脏功能及构型改变也会越显著.采用超声心动图对GDM胎儿心脏功能评价具有重要价值.

목적:관찰초성심동도응용우임신기당뇨병태인심공능중적평개효과.방법:선취아원2012년1월~2014년8월기간90례임신기당뇨병(GDM)잉부,안조기혈당수평분조위갑、을조각45례;이동기건강단태임신잉부45례위대조조.병채용초성심동도검측태인심장폐동맥판배내경(PVA)、심장이첨판배내경(MVA)、주동맥판배내경(AVA)、삼첨판배내경(TVA)、좌/우실서장말기내경(LVDd、RVDd)、좌/우실벽후도(LVWT、RVWT)、좌/우실수축말기내경(LVDs、RVDs)등.결과:을조태인TVA、LVMPI、MVA、RVMPI급PVA균명현대우대조조,P<0.05;갑조TVA、PVA、MVA、AVA급RVMPI균명현소우을조,P<0.05.갑조각판배내경여대조조비교,P>0.05;이기LVMPI급RVMPI균명현대우대조조,P<0.05.결론:잉부혈당수평월고,태인심장공능급구형개변야회월현저.채용초성심동도대GDM태인심장공능평개구유중요개치.
Objective: Observation applies gestational diabetes echocardiography in the evaluation of fetal cardiac function results. Methods:Interception of our hospital in January 2012~August 2014 period 90 cases of gestational diabetes (GDM) in pregnant women according to their blood sugar levels are grouped into A,B,all 45 cases;and choose healthy singleton pregnancies the same period 45 cases of the control group.And the use of echocardiography to detect fetal heart inside diameter of the pulmonary valve annulus (PVA),the heart of the inner diameter of the mitral annulus (MVA),the inner diameter of the aortic annulus (AVA),the inner diameter of the tricuspid annulus (TVA),left/right ventricular end-diastolic inside diameter (LVDd,RVDd),left/right ventricular wall thickness (LVWT, RVWT), left/right ventricular systolic diameter (LVDs,RVDs) and so on.Results:B fetus EVA,LV MPI,MVA,RMPI and PVA were significantly higher than the control group,P<0.05;Group A TVA, PVA,MVA,AVA and RVMPI were significantly lower than group B,P<0.05.Group A each annulus inner diameter compared with control group,P>0.05;its LVMPI and RVMPI were significantly higher than the control group,P<0.05.Conclusion:The higher the blood sugar levels in pregnant women, fetal cardiac function and configuration change will be more significant.However,the use of echocardiography has an important value for GDM fetal cardiac function evaluation.
