目的:通过比较第5天冷冻囊胚解冻培养4h与过夜培养20h后移植的临床结局,探讨囊胚解冻后到移植用以培养的适宜时间。方法:在第5天冷冻囊胚解冻移植周期中,按照囊胚解冻后到移植用以培养的时间不同分为2组:解冻4h后移植组(A组,移植周期157个,囊胚268枚)和提前解冻过夜培养20h后移植组(B组,移植周期227个,囊胚370枚),比较其妊娠结局。结果:B组的囊胚孵化率明显高于A组,差异有统计学意义(P<0.01);B组周期取消率稍高于A组,但差异无统计学意义(P>0.05);B组在平均移植胚胎个数上稍少于A组,但差异无统计学意义(P>0.05);A、B2组在临床妊娠率、胚胎种植率、流产率和多胎妊娠率上差异无统计学意义(P>0.05)。结论:囊胚提前解冻过夜培养20h与培养4h后移植的临床结局无明显差异,操作人员对于囊胚解冻时间上将有更多选择。

Objective:To compare the pregnancy outcomes when frozen blastocyst was transferred 4 hours after being thawed or 20 hours after being cultured overnight. Methods:For the 5 days freezing thawing blasto-cysts transplantation cycle, all the cycles are divided into two groups based on different culture time before trans-fer, those blastocysts transferred 4 hours after being thawed were labeled as A group (n=157) and those cultured overnight before being transferred the next day were included in B group (n=227). The pregnancy outcomes were compared between the two groups. Results:The proportion of thawing cycles leading to no transfer did not differ between groups (1.2%versus 2.2%for A and B, respectively). However, the proportion of hatched blastocysts de-rived from B group was signiifcantly higher than that from A group (41.7%versus 86.5%, respectively, P<0.001). All differences of the clinical pregnancy rate, implantation rate, miscarriage rate and multiple pregnancy rate between two groups have not yet reached the statistical signiifcance. Conclusion:Overnight culture of thawed blastocyst before transfer does not affect the main pregnancy outcomes. For blastocyst thawing time, operators will have more choices.
