目的：探讨晚期非小细胞肺癌患者的营养状况及营养支持对患者的临床疗效和营养指标的影响。方法：选取我院收治的晚期非小细胞肺癌住院患者60例，随机分为营养支持组和对照组，均采用 TP 方案进行化疗。在化疗前应用 NRS －2002量表进行营养风险评估，在化疗过程中对营养支持组的患者进行饮食指导和营养支持；而对照组则不给予任何营养指导和支持。比较2组患者的近期疗效、毒副反应和化疗前、后营养指标的变化。结果：晚期非小细胞肺癌住院患者营养风险的发生率为61．7％。和对照组比较，营养支持组患者的化疗有效率无显著差异，但化疗后的骨髓抑制发生率显著低于对照组，差异有统计学意义（P ＜0．05）。治疗前，营养支持组和对照组患者的体质量指数（BMI）、白蛋白（ALB）、血红蛋白（Hb）和前白蛋白（PA）等营养指标均无显著差异。治疗后，营养支持组患者的 BMI、ALB、Hb 和 PA 等营养指标均有所改善，但对照组患者仅有 ALB 有所改善；并且，治疗后营养支持组患者的各项营养指标均优于对照组，差异有统计学意义（P ＜0．05）。结论：晚期非小细胞肺癌患者的营养风险发生率高。营养支持可明显地改善晚期非小细胞癌患者的营养状况，减轻其化疗毒副反应，提高化疗耐受性。
목적：탐토만기비소세포폐암환자적영양상황급영양지지대환자적림상료효화영양지표적영향。방법：선취아원수치적만기비소세포폐암주원환자60례，수궤분위영양지지조화대조조，균채용 TP 방안진행화료。재화료전응용 NRS －2002량표진행영양풍험평고，재화료과정중대영양지지조적환자진행음식지도화영양지지；이대조조칙불급여임하영양지도화지지。비교2조환자적근기료효、독부반응화화료전、후영양지표적변화。결과：만기비소세포폐암주원환자영양풍험적발생솔위61．7％。화대조조비교，영양지지조환자적화료유효솔무현저차이，단화료후적골수억제발생솔현저저우대조조，차이유통계학의의（P ＜0．05）。치료전，영양지지조화대조조환자적체질량지수（BMI）、백단백（ALB）、혈홍단백（Hb）화전백단백（PA）등영양지표균무현저차이。치료후，영양지지조환자적 BMI、ALB、Hb 화 PA 등영양지표균유소개선，단대조조환자부유 ALB 유소개선；병차，치료후영양지지조환자적각항영양지표균우우대조조，차이유통계학의의（P ＜0．05）。결론：만기비소세포폐암환자적영양풍험발생솔고。영양지지가명현지개선만기비소세포암환자적영양상황，감경기화료독부반응，제고화료내수성。
Objective:To investigate the nutritional status of patients with advanced non -small cell lung cancer (NSCLC)and the effect of nutritional support on the efficiency and toxicity of chemotherapy and nutrient indicators. Methods:Totally 60 patients with advanced NSCLC were randomly divided into nutritional support group and control group with TP chemotherapy.NRS -2002 was used to perform nutritional risk screening before chemotherapy.The nu-tritional support group were given the nutritional support during chemotherapy,while the control group was given clini-cal routine treatments.Compared the efficiency and toxicity of chemotherapy and the changes of nutrient indicators. Results:The incidence of nutritional risk of hospitalized patients with advanced NSCLC was 61.7%.Compared with the control group,the effective rate of chemotherapy of the nutritional support group had no significant difference.The incidence of bone marrow suppression was significantly lower in the nutritional support group than that of the control group.There was no significant difference of nutrient indicators such as BMI,ALB,Hb and PA between two groups be-fore chemotherapy.The nutrient indicators of BMI,ALB,Hb and PA in the nutritional support group were improved af-ter chemotherapy,while only ALB was improved in the control group.The nutrient indicators of the nutritional support group were significantly better than the control group after treatment.Conclusion:Patients with advanced NSCLC have a high incidence of nutrition risk.Nutritional support can improve the nutritional status of patients with advanced NSCLC and reduce the toxicity of chemotherapy.
