目的：探讨改良 R．E．N．A．L．评分系统在 T1期肾癌患者保留肾单位手术中的临床价值。方法：回顾性分析2008年1月－2013年12月 T1期肾癌患者48例应用改良 R．E．N．A．L．肾脏测量评分系统进行量化评分，并进行应用评价。结果：应用改良 R．E．N．A．L．评分系统将48例 T1期肾癌患者分为高值9例（A 组），中值26例（B 组），低值13例（C 组），A、B、C 三组在手术方式、热缺血时间、术中出血量、手术时间均存在明显差异（均 P ＜0．01），而肌酐上升值、住院时间无明显差异（P ＞0．05）。结论：改良 R．E．N．A．L．评分系统可以较好地对 T1期肾癌保留肾单位手术进行评估，准确评价了肾癌保留肾单位手术的难度，可作为评估手术难度分级标准，为选择手术方式提供参考。

Objective:To explore the evaluation of the modified R.E.N.A.L.nephrometry score in nephron spar-ing surgery.Methods:Between January 2008 and December 2013,48 cases of T1 stage renal cancer were reviewed on the clinical data,who were assessed with the modified R.E.N.A.L.nephrometry score.Results:All 48 cases were di-vided into three groups,9 high -score (group A),26 moderated -score (group B)and 13 low -score (group C)by the modified R.E.N.A.L.nephrometry score.Significant differences were seen in surgical approach,warm ischemia time,operating time and estimated blood loss(All P <0.01),but no significant differences were seen in percentage change of the serum creatinine level and length of stay(P >0.05).Conclusion:The modified R.E.N.A.L.nephrom-etry score can accurately reflects the characteristics of T1 renal cell carcinoma undergoing nephron sparing surgery, and can be used as operation difficulty grading standards,provide the reference for selecting surgical approach.
