目的:探讨降钙素原检测在儿童感染性疾病中的应用价值。方法取我院2012年3月~2013年9月收治的儿科感染患者452例为研究对象,分为细菌性感染组267例,(细菌感染组又根据病情严重程度分为局部细菌感染组178例及重症细菌感染组89例)病毒性感染组185例,另取正常体检儿童70例作为对照组,对各组儿童均分别检测PCT、CRP、WBC并对结果进行分析与比较。结果267例细菌性感染组PCT、CRP、WBC明显高于对照组与病毒性感染组,差异非常显著(P<0.01);185例病毒性感染组PCT、CRP、WBC与对照组无显著差异(P>0.05);PCT诊断重症感染、局部细菌感染的敏感性分别为100%(89/89)、41.6%(74/178);CRP诊断重症感染、局部细菌感染的敏感性分别为82%(73/89)、31.4%(56/178);WBC诊断重症感染、局部细菌感染的敏感性分别为64%(73/89)、36.0%(64/178)。PCT、CRP、WBC诊断特异性分别为89%、76.5%、74.1%。结论 PCT检测在儿童感染性疾病的鉴别诊断及预后判断和指导临床合理应用抗菌素具有重要的临床意义。

Objective To investigate the clinical application value of the serum PCT in children infectious diseases.Methods Totally 452 hospitalized children with infectious diseases were enrolled in the study from Mar 2012-Sep 2013 in hospital .These children were divided into the bacterial infection group 267 cases and the virus infection group 185 cases ,while 70 non-infection cases were set as control.PCT,CRP, WBC were detected simultaneously.Results PCT,CRP and WBC are significantly higher in the bacterial infection group than that of virus infection group and non-infection group (P<0.01),while there existed no different between virus infection and non~infection group (P>0.05). The sensitivity of PCT for the diagnosis of severe bacterial infection is 100%, (89/89)while local bacterial infection is 41.6%(74/178),The sensitivity of CRP for the diagnosis of severe bacterial infection is 82%,(73/89),while local bacterial infection is 31.4%(56/178) ,The sensitivity of WBC for the diagnosis of severe bacterial infection is 64%, (73/89),while local bacterial infection is 36%(64/178).Conclusion PCT is a highly efficient laboratory parameter for the diagnosis of bacterial infection in children .
