目的:探讨收缩期二尖瓣环位移(systolic mitral annular displacement,MADs)评价维吾尔族(维)、汉族两民族间冠心病患者左心长轴收缩功能改变的价值。方法对确诊的冠心病患者分两组,维吾尔族25例,汉族30例,分析两组间临床资料、生化指标、冠脉病变、常规心脏超声测值及MADs测量指标,MADs指标有室间隔、侧壁、前壁、下壁、前室间隔、后壁二尖瓣环6个节点收缩期最大位移值及6个节点均值等。结果维吾尔族冠心病患者较汉族年龄偏大;冠脉病变中维吾尔族患者单支病变较汉族少,多支病变多于汉族;维、汉族患者两组之间三腔心前室间隔、后壁及四腔心室间隔3个位点二尖瓣环收缩期最大位移差异有统计学意义(P<0.05),维吾尔族低于汉族;6个位点均值差异有统计学意义(P<0.05),维吾尔族低于汉族;维吾尔族与汉族6个位点均值与LVEF均呈正相关。结论维吾尔族及汉族左室长轴局部及整体收缩功能均有差异,MADs可以敏感地检测到左心室长轴收缩功能异常,从而识别高危患者,均值是较理想的指标。

목적:탐토수축기이첨판배위이(systolic mitral annular displacement,MADs)평개유오이족(유)、한족량민족간관심병환자좌심장축수축공능개변적개치。방법대학진적관심병환자분량조,유오이족25례,한족30례,분석량조간림상자료、생화지표、관맥병변、상규심장초성측치급MADs측량지표,MADs지표유실간격、측벽、전벽、하벽、전실간격、후벽이첨판배6개절점수축기최대위이치급6개절점균치등。결과유오이족관심병환자교한족년령편대;관맥병변중유오이족환자단지병변교한족소,다지병변다우한족;유、한족환자량조지간삼강심전실간격、후벽급사강심실간격3개위점이첨판배수축기최대위이차이유통계학의의(P<0.05),유오이족저우한족;6개위점균치차이유통계학의의(P<0.05),유오이족저우한족;유오이족여한족6개위점균치여LVEF균정정상관。결론유오이족급한족좌실장축국부급정체수축공능균유차이,MADs가이민감지검측도좌심실장축수축공능이상,종이식별고위환자,균치시교이상적지표。
Objective To observe the effect of systolic mitral annular displacement (MADs) on left ventricular longitudinal systolic function between Uygur and Han patients undergoing coronary heart disease. Methods Patients who were diagnosed with coronary heart disease were divided into two groups, one group included 25 cases of Uygur patients, the other one included 30 cases of Han patients. Biochemical indicators, coronary artery lesions, routine cardiac ultrasound measurements and MADs which indicators included septal, lateral, anterior, inferior, front septal, posterior six MADs and average values were analyzed. Results Uygur CHD patients were older than the Han patients in two groups;Single vessel disease of Uygur coronary lesions was less than Han, but multivessel disease was more than Han patients;Three MADs of front septal, posterior ,septal were significantly different between two groups (P<0.05);Uygur MADs value decreased compared with Han patients; The average values of six sites were all different between two groups (P <0.05); Uygur MADs decreases compared with Han patients;The average values were positively correlated with LVEF between two groups. Conclusion The local and globel left ventricular longitudinal systolic function were different between two groups. Left ventricular systolic function of longitudinal can be sensitively detected by MADs. The average was an ideal target to identify high-risk patients.
