目的：分析肺结节病误诊患者的临床特点及辅助检查结果，提高临床肺结节病的诊断率。方法收集2014年2月~12月呼吸专科门诊诊断为肺结节病患者25例，这些患者曾在外院未诊断出结节病，记录并分析这些患者临床表现特点以及辅助检查结果，分析可能导致误诊的原因。结果肺结节病临床表现以呼吸系统受累症状常见，多被误诊为肺部感染性疾病或肿瘤。肺结节病可累及肺外脏器表现，对疾病认识不足和组织取材有限以及病理检查缺乏是误诊的主要原因。结论临床工作中应提高对肺结节病多系统受累表现的认识，完善并提高辅助检查手段可降低肺结节病降低误诊率。

Objective To analyze the clinical characteristics and auxiliary examination results of misdiagnosis patients with pulmonary sarcoidosis. Methods 25 cases who were diagnosed with pulmonary sarcoidosis from February 2014 to December 2014 in respiratory out-patient department were collected. These patients were misdiagnosed in other centers. Clinical data and auxiliary examination results were collected and analyzed to find the causes of misdiagnosis. Results Respiratory symptoms were common system involved in misdiagnosed sarcoidosis patients, who were mostly misdiagnosed as pulmonary infectious diseases or cancer. Pulmonary sarcoidosis usually affected the clinical characteristics of extrapulmonary organs. neglection the diagnosis of diseases, nonproficient bronchoscopy biopsy and lack of histology confirmations were the main causes of misdiagnosis. Conclusion Improvements on the knowledge of the clinical characterisitics and multisystem involvements in pulmonary sarcoidosis may improve the diagnosis skills in clinical practice. Meanwhile combining with appropriate auxiliary examination may reduce the misdiagnosis.
