目的:探讨抗谷氨酸脱羧酶抗体(GAD-Ab)、抗酪氨酸磷酸酶抗体(IA2-Ab)和抗胰岛细胞抗体(ICA)在糖尿病患者临床分型中的应用价值。方法选取2012年1月—2014年12月该院收治的糖尿病确诊病例128例,其中1型糖尿病48例病例1组,2型糖尿病组80例为病例2组,同时另选同期在该院进行健康检查的健康人士60例作为对照组,采用酶联免疫吸附试验(ELISA)检测GAD-Ab、IA2-Ab,采用间接免疫荧光法检测ICA,观察比较三组间GAD-Ab、IA2-Ab和ICA阳性率。结果病例1组GAD-Ab、IA2-Ab、ICA阳性率分别为62.50%、12.50%、22.92%,明显高于病例2组5.00%、1.25%、3.75%和对照组0%、0%、0%,比较差异均具有统计学意义(P<0.05);其中GAD-Ab对1型糖尿病的敏感性明显高于IA2-Ab和IAC,比较差异有统计学意义(P<0.05),而联合检测阳性率97.92%又明显高于GAD-Ab单项检测,比较差异有统计学意义(P<0.05)。结论GAD-Ab对1型糖尿病具有较高敏感度,而GAD-Ab、IA2-Ab和IAC联合检测结果判定更为准确,对临床糖尿病患者早期准确分型具有重要意义。

목적:탐토항곡안산탈최매항체(GAD-Ab)、항락안산린산매항체(IA2-Ab)화항이도세포항체(ICA)재당뇨병환자림상분형중적응용개치。방법선취2012년1월—2014년12월해원수치적당뇨병학진병례128례,기중1형당뇨병48례병례1조,2형당뇨병조80례위병례2조,동시령선동기재해원진행건강검사적건강인사60례작위대조조,채용매련면역흡부시험(ELISA)검측GAD-Ab、IA2-Ab,채용간접면역형광법검측ICA,관찰비교삼조간GAD-Ab、IA2-Ab화ICA양성솔。결과병례1조GAD-Ab、IA2-Ab、ICA양성솔분별위62.50%、12.50%、22.92%,명현고우병례2조5.00%、1.25%、3.75%화대조조0%、0%、0%,비교차이균구유통계학의의(P<0.05);기중GAD-Ab대1형당뇨병적민감성명현고우IA2-Ab화IAC,비교차이유통계학의의(P<0.05),이연합검측양성솔97.92%우명현고우GAD-Ab단항검측,비교차이유통계학의의(P<0.05)。결론GAD-Ab대1형당뇨병구유교고민감도,이GAD-Ab、IA2-Ab화IAC연합검측결과판정경위준학,대림상당뇨병환자조기준학분형구유중요의의。
Objective To investigate the anti glutamic acid decarboxylase antibody (GAD AB), anti tyrosine phosphatase antibody (IA2 AB) and anti islet cell antibodies (ICA) in patients with diabetes mellitus (DM) type of application value. Methods Select January 2012 - 2014 December in our hospital were diagnosed with diabetes 128 cases of, the type I diabetes 48 cases in group 1 and type II diabetes group 80 cases for cases of the two groups. At the same time in the same period in our hospital were health examination of health professionals in 60 cases do as control group, using enzyme linked immunosor-bent assay (ELISA) for the detection of GAD AB, IA2 AB, using indirect immunofluorescence assay for detection of ICA observed were compared among the three groups GAD AB, IA2 AB and ICA positive rate. Results The positive rates of I-A2-Ab, ICA and GAD-Ab were 62.50%, 12.50%, 22.92%, 5%, 0%, 3.75%, 1.25%, 0%and 0%, respectively, and the dif-ference was statistically significant (P<0.05). The positive rate of GAD-Ab was significantly higher than that of IA2-Ab and IAC (P<0.05), and the difference was statistically significant (GAD-Ab). Conclusion GAD-Ab has a high sensitivity to type I diabetes, and the combined detection of IAC, IA2-Ab and GAD-Ab is more accurate, and it has important significance for early and accurate classification of clinical diabetes.
