胃食管反流性咳嗽（GERC）是因胃酸或其他胃内容物反流进入食管，形成以咳嗽为主要表现的临床综合征，属于胃食管反流病的一种特殊类型。其机制涉及微量误吸、食管-支气管反射、食管运动功能失调、植物神经功能失调与气道神经源性炎症等。GERC 临床可见慢性咳嗽、干咳、无痰，平卧或饱食后症状加重，咽部异物感、咽痛，甚则声音嘶哑、哮喘，多伴有反酸、反食、胸骨后疼痛、上腹痛、腹胀等消化道症状。胃食管反流性咳嗽属中医学中内伤咳嗽范畴，从肺胃相关角度，认为其基本病机为胃虚气逆、肺失清肃，临证应用和胃降逆法的代表方旋覆代赭石汤随症加减治疗，多获良效。通过对胃食管反流性咳嗽临床治验的归纳总结，体现和印证了“病症结合、方证相应”中医辨证体系与“异病同治、异病同治”中医治则的特色和优势。

Gastroesophageal reflux cough is a common clinical disease,also is one of the main cau-ses of chronic cough. According to years of clinical experience,gastroesophageal reflux cough is believed to belong to the category of internal cough. It is based on the lung and stomach theory that the basic pathogene-sis of this disease is the deficiency of stomach qi and the impaired lung depuration. Xuanfudaizheshi decoc-tion is the representative prescription of harmonizing stomach for descending adverse qi method,and its appli-cation in the treatment of Gastroesophageal reflux cough is very good. The purpose of this article is to reflect the characteristics and advantages of the treatment based on syndrome differentiation and homotherapy for het-eropathy principle,by summarizing my experience in clinical diagnosis and treatment.
