[目的]探讨连续护理干预对老年慢性病病人健康知识掌握程度及自理能力的改善效果。[方法]2014年2月—2015年1月50例老年慢性病病人在出院后接受连续护理干预，作为干预组，选取2013年2月—2014年1月出院后仅接受常规护理的50例老年慢性病病人作为对照组。护理前后统计两组病人的饮食知识、疾病知识、防治措施以及自理能力，包括运动、健康责任、药物治疗和压力应对等项目得分。[结果]两组干预前饮食知识、疾病知识、防治措施、运动、健康管理、药物治疗和压力应对得分相比，差异无统计学意义(P>0.05)；干预后干预组饮食知识、疾病知识、防治措施分别为(28.3分±3.5分)、(30.4分±1.5分)和(31.2分±0.8分)，对照组为(22.5分±2.7分)、(24.1分±2.4分)和(25.4分±1.2分)，两组比较差异有统计学意义(P<0.05)；干预后干预组运动、健康责任、药物治疗和压力应对得分分别为(22.9分±1.8分)、(22.6分±1.5分)、(21.8分±2.4分)和(23.2分±1.4分)，对照组为(18.3分±1.5分)、(17.8分±2.1分)、(18.1分±2.2分)和(16.4分±2.0分)，差异有统计学意义(P<0.05)。[结论]连续护理干预能有效提升老年慢性病病人的健康知识，改善自理能力，有助于病人症状的改善。

Objective:To probe into the improvement effect of continuous nursing intervention on health knowl-edge mastering and self care ability of elderly patients with chronic diseases.Methods:A total of 50 elderly pa-tients with chronic diseases after discharging from February 2014 to January 2015 received the continuous nurs-ing intervention and they were taken as the intervention group;while another 50 cases of discharging patients were selected as control group from February 2013 to January 2014 and they only received the routine nursing. The scores of dietary knowledge,disease related knowledge,prevention measures and self care ability were counted which included the sports,health responsibility,medication therapy and stress coping in two groups be-fore and after the nursing.Results:There was no statistically significant difference in scores of dietary knowl-edge,disease related knowledge,prevention measures,sports,health management,medication therapy and stress coping between both groups before the intervention (P>0.05 );after the intervention the scores of dietary knowledge,disease related knowledge and prevention measures in intervention group respectively were (28.3± 3.5),(30.4±1.5)and (31.2±0.8),while those in control group respectively were (22.5±2.7),(24.1±2.4) and (25.4±1.2),the difference was statistically significant (P<0.05);after the intervention the scores of sports,health responsibility,medication therapy and stress coping respectively were (22.9±1.8),(22.6±1.5), (21.8±2.4)and (23.2±1.4),while those in control group respectively were (18.3±1.5),(17.8±2.1),(18.1± 2.2)and (16.4±2.0),and the difference was statistically significant (P<0.05).Conclusion:The continuous nursing intervention can effectively improve the health knowledge of elderly patients with chronic diseases and improve their self care ability,thus help to improve their symptoms.
