目的：观察指固有动脉侧方掌侧皮瓣修复糖尿病患者合并指端缺损的临床效果。方法收集近年来某医院治疗的32例糖尿病合并指端缺损患者为研究对象，所有患者均为指固有动脉侧方掌侧皮瓣手术修复，围术期应用胰岛素控制血糖，观察治疗效果。结果所有患者皮瓣全部成活，供区创面Ⅰ期愈合，围术期患者血糖在适宜范围。术后随访6~12个月，伤指术后皮瓣质地、厚度、色泽与受区相似，不臃肿、耐磨，手指伸屈功能满意，外形美观，皮瓣两点分辨觉10~15 mm，感觉良好。结论指固有动脉侧方掌侧皮瓣手术修复糖尿病合并指端缺损患者可在局麻下进行，操作简单，安全可靠，皮瓣切取后供区损伤小可以直接缝合，必须保证患者围术期血糖在适宜范围，治疗效果较为满意，值得临床推广应用。
목적：관찰지고유동맥측방장측피판수복당뇨병환자합병지단결손적림상효과。방법수집근년래모의원치료적32례당뇨병합병지단결손환자위연구대상，소유환자균위지고유동맥측방장측피판수술수복，위술기응용이도소공제혈당，관찰치료효과。결과소유환자피판전부성활，공구창면Ⅰ기유합，위술기환자혈당재괄의범위。술후수방6~12개월，상지술후피판질지、후도、색택여수구상사，불옹종、내마，수지신굴공능만의，외형미관，피판량점분변각10~15 mm，감각량호。결론지고유동맥측방장측피판수술수복당뇨병합병지단결손환자가재국마하진행，조작간단，안전가고，피판절취후공구손상소가이직접봉합，필수보증환자위술기혈당재괄의범위，치료효과교위만의，치득림상추엄응용。
Objective Clinical observation of renovate effect of diabetes mellitus fingertip with finger palmar artery side flap. Methods In recent years, a hospital for treatment of 32 cases of diabetic patients with fingertip defect patients in this study, all patients were finger palmar artery side flap surgical repair, perioperative insulin to control blood sugar, therapeu-tic effects were observed. Results All patients survived the flap donor site Ⅰ wound healing, blood around the surgical pa-tients in an appropriate range. Patients were followed up 6-12 months, the injured finger flaps texture, thickness, color and subject area similar not bloated, wear, finger flexion function satisfaction, appearance, point discrimination flap 10~15mm, feel good. Conclusion diabetes mellitus fingertip with finger palmar artery side flap can be performed under local anesthesi-a, the operation is simple, safe and reliable for the region after the flap was little damage can be sutured directly, we must ensure that patients perioperative glucose in the appropriate range, the treatment effect is satisfactory, worthy of clinical ap-plication.
