目的：总结合并巨大左心室心脏瓣膜病的外科治疗经验。方法对2008年4月－2014年6月施行的56例巨大左心室瓣膜置换术的临床资料进行回顾性分析。结果术后发生各种并发症33例（58．92％），死亡3例（5．35％）。恶性室性心律失常和低心排综合征及其所导致的多脏器功能衰竭是合并巨大左心室瓣膜置换术后的主要并发症和致死原因。结论合并巨大左心室的心脏瓣膜病术后并发症和死亡率均较高。完善的术前准备，充分的术中心肌保护，严密的术后监护治疗是提高合并巨大左心室的心脏瓣膜病患者手术疗效的关键。
목적：총결합병거대좌심실심장판막병적외과치료경험。방법대2008년4월－2014년6월시행적56례거대좌심실판막치환술적림상자료진행회고성분석。결과술후발생각충병발증33례（58．92％），사망3례（5．35％）。악성실성심률실상화저심배종합정급기소도치적다장기공능쇠갈시합병거대좌심실판막치환술후적주요병발증화치사원인。결론합병거대좌심실적심장판막병술후병발증화사망솔균교고。완선적술전준비，충분적술중심기보호，엄밀적술후감호치료시제고합병거대좌심실적심장판막병환자수술료효적관건。
Objective To summarize the experience of surgical treatment for severe heart valve dis -ease with giant left ventricle.Method 56 patients with giant left ventricle underwent valve replacement operation in our hospital from April 2008 to June 2014 were analyzed retrospectively.Results The rate of complication was 58.92% of 33 cases and the death rate was 5.35% of 3 cases.The main complication and causes of death of replacement surgery of giant left ventricle were severe arrhythmia, low cardiac out-put and multiple organ failure and the caused multiple organ failure.Conclusion The mortality and the rate of complication for the patients with giant left ventricle are higher.Sufficient preoperative prepara-tion, good myocardial protection and intensive postoperative cares are the keys to improve the effect of surgical treatment for patients of heart valve disease with giant left ventricle.
