目的：观察维生素 E（Vit E）治疗精神分裂症伴随的迟发性运动障碍（TD）的效果及可能的机制。方法将80例 TD 患者随机分成两组，每组40例，分别采用安慰剂（对照组）、Vit E（治疗组，剂量为1200IU/ d）治疗12周。在治疗始末分别进行阳性和阴性症状量表（PANSS）、异常不自主运动量表（AIMS）评定；并予分光光度法测定治疗前后血浆超氧化物歧化酶（ SOD）、谷胱苷肽过氧化物酶（ GSH - Px）、过氧化氢酶（ CAT）活性及丙二醛（MDA）水平。结果①对照组治疗前后 AIMS 单项分及总评分比较，差异无统计学意义（P >0.05）；治疗组 AIMS除表情肌、全身单项评分外，其余各项分值及总评分均下降（P <0.05）。②Vit E 可升高 SOD 活性[治疗前（85.8±18.3）U/ ml、治疗后（101.5±25.1）U/ ml]、CAT 活性[治疗前（1.8±1.1）U/ ml、治疗后（3.1±2.1）U/ ml]、GSH - Px活性[治疗前（106.8±28.3）U/ ml、治疗后（133.1±31.0）U/ ml]，降低 MDA 水平[治疗前（13.2±6.7）nmol/ ml、治疗后（8.6±5.5）nmol/ ml]（P <0.05）；安慰剂则对上述指标无明显影响（P >0.05）。结论 Vit E 能有效改善 TD症状，氧化应激可能是 TD 发病与演进的重要因素。
목적：관찰유생소 E（Vit E）치료정신분렬증반수적지발성운동장애（TD）적효과급가능적궤제。방법장80례 TD 환자수궤분성량조，매조40례，분별채용안위제（대조조）、Vit E（치료조，제량위1200IU/ d）치료12주。재치료시말분별진행양성화음성증상량표（PANSS）、이상불자주운동량표（AIMS）평정；병여분광광도법측정치료전후혈장초양화물기화매（ SOD）、곡광감태과양화물매（ GSH - Px）、과양화경매（ CAT）활성급병이철（MDA）수평。결과①대조조치료전후 AIMS 단항분급총평분비교，차이무통계학의의（P >0.05）；치료조 AIMS제표정기、전신단항평분외，기여각항분치급총평분균하강（P <0.05）。②Vit E 가승고 SOD 활성[치료전（85.8±18.3）U/ ml、치료후（101.5±25.1）U/ ml]、CAT 활성[치료전（1.8±1.1）U/ ml、치료후（3.1±2.1）U/ ml]、GSH - Px활성[치료전（106.8±28.3）U/ ml、치료후（133.1±31.0）U/ ml]，강저 MDA 수평[치료전（13.2±6.7）nmol/ ml、치료후（8.6±5.5）nmol/ ml]（P <0.05）；안위제칙대상술지표무명현영향（P >0.05）。결론 Vit E 능유효개선 TD증상，양화응격가능시 TD 발병여연진적중요인소。
Objective To evaluate the therapeutic effects of vitamin E(Vit E)on the AIMS scores in schizophrenic patients with tar-dive dyskinesia(TD)and possible therapeutic mechanisms. Methods Eligible patients(n =80)would be randomized to receive vitamin E (1 200U/ d)or placebo. The present study would be double blinded and last for 12 weeks. The symptoms of TD and psychopathology were as-sessed by using Abnormal Involuntary Movement Scale(AIMS)and Positive and Negative Symptoms Scale(PANSS)respectively,the plasma superoxide dismutase(SOD),catalase(CAT),glutathione peroxidase(GSH - Px)activities and maleicdialdehyde(MDA)levels were assayed. Results ①Vit E could be markedly effective in reducing the severities of dyskinesia in patients of TD,but the placebo could not improve the AIMS scores. ②The Vit E might increase the activities of SOD[baseline(85. 8 ±18. 3)U/ ml,after treatment(101. 5 ±25. 1)U/ ml], CAT[baseline(1.8 ±1.1)U/ ml,after treatment(3.1 ±2.1)U/ ml],GSH - Px[ baseline(106.8 ±28.3)U/ ml,after treatment(133.1 ± 31.0)U/ ml]and decrease the levels of MDA[baseline(13.2 ±6. 7)nmol/ ml,after treatmDent(8. 6 ± 5. 5)nmol/ ml]significantly(P <0.05),however,the placebo could not effect on the biochemistry measures. Conclusion Vit E is markedly effective on reducing the severity of dyskinesia in TD,and oxidative damages might be key pathophysiological factor of TD.
