目的:探讨血清N-末端脑钠肽前体(NT-proBNP)、心肌肌钙蛋白I2(cTnI2)、肌红蛋白(MYO)、肌酸激酶同工酶(CK-MB)水平与老年慢性心力衰竭(CHF)患者心功能分级的关系。方法选取2012年5月~2015年1月我院收治的确诊为老年CHF患者289例作为观察组,选取同期来我院进行健康体检的老年志愿者100例作为对照组,观察两组血清NT-proBNP、cTnI2、MYO、CK-MB检测结果。结果观察组患者NT-proBNP、MYO水平均显著高于对照组,cTnI2、CK-MB水平显著低于对照组,差异有统计学意义(P<0.05);随着患者心功能分级的升高,观察组NT-proBNP、cTnI2、CK-MB、MYO指标不断增加,差异有统计学意义(P<0.05)。结论血清NT-proBNP是评价老年CHF患者心功能的可靠指标,心肌损伤标志物cTnI2、CK-MB、MYO与心肌损伤严重程度有一定的关系,临床可通过检测机体NT-proBNP、cTnI2、CK-MB、MYO水平预测疾病的发生。

목적:탐토혈청N-말단뇌납태전체(NT-proBNP)、심기기개단백I2(cTnI2)、기홍단백(MYO)、기산격매동공매(CK-MB)수평여노년만성심력쇠갈(CHF)환자심공능분급적관계。방법선취2012년5월~2015년1월아원수치적학진위노년CHF환자289례작위관찰조,선취동기래아원진행건강체검적노년지원자100례작위대조조,관찰량조혈청NT-proBNP、cTnI2、MYO、CK-MB검측결과。결과관찰조환자NT-proBNP、MYO수평균현저고우대조조,cTnI2、CK-MB수평현저저우대조조,차이유통계학의의(P<0.05);수착환자심공능분급적승고,관찰조NT-proBNP、cTnI2、CK-MB、MYO지표불단증가,차이유통계학의의(P<0.05)。결론혈청NT-proBNP시평개노년CHF환자심공능적가고지표,심기손상표지물cTnI2、CK-MB、MYO여심기손상엄중정도유일정적관계,림상가통과검측궤체NT-proBNP、cTnI2、CK-MB、MYO수평예측질병적발생。
Objective To explore the relation of NT-proBNP,cTnI2,MYO,CK-MB with cardiac function classification in elderly patients with chronic heart failure. Methods 289 cases of patients with CHF from May 2012 to January 2015 in our hospital were chosen as the observation group,100 cases of healthy check-up of elderly volunteers in the same term were chosen as the control group.The levels of NT-proBNP,cTnI2,MYO,CK-MB were analyzed. Results The levels of NT-proBNP,MYO in the observation group were higher than those in the control group;the levels of cTnI2,CK-MB in the observation group were lower than those in the control group,there were significant differences between the two groups (P<0.05);with the increase of heart function classification,the levels of NT-proBNP,cTnI2,MYO,CK-MB in-creased continuously (P<0.05). Conclusion NT-proBNP is the evaluation of the reliability index of the heart function in elderly patients with CHF,cTnI2,CK-MB,MYO has connection with heart function classification.It can predict the CHF by detecting the levels of NT-proBNP,cTnI2,CK-MB,MYO.
