目的 探讨阑尾黏液囊肿的诊断、手术治疗方法及临床效果. 方法 回顾性分析2007年4 月至2013年9月吉林油田总医院收治的6例阑尾黏液囊肿患者的临床资料,结合相关文献对阑尾黏液囊肿的诊断及手术治疗方法进行总结.结果 本研究患者术前误诊4例,误诊率为66.7%. 囊肿位于阑尾远端4例,中远端2例,形态呈椭圆形腊肠样,内容物为黄白色胶冻样物,4例常规切除阑尾、系膜及囊肿,2例行回盲部切除及回结肠端端吻合术. 术后病理诊断阑尾黏液囊肿4例,阑尾黏液潴留2例,术后无明显并发症发生,随访1~3年未见囊肿复发及转移. 结论 阑尾黏液囊肿临床表现不典型,术前误诊率极高,临床医生应提高对本病的认识,术中细致操作,对恶变可疑者及时快速冰冻病理切片检查,根据病例结果选择合适术式,可提高治疗效果.
目的 探討闌尾黏液囊腫的診斷、手術治療方法及臨床效果. 方法 迴顧性分析2007年4 月至2013年9月吉林油田總醫院收治的6例闌尾黏液囊腫患者的臨床資料,結閤相關文獻對闌尾黏液囊腫的診斷及手術治療方法進行總結.結果 本研究患者術前誤診4例,誤診率為66.7%. 囊腫位于闌尾遠耑4例,中遠耑2例,形態呈橢圓形臘腸樣,內容物為黃白色膠凍樣物,4例常規切除闌尾、繫膜及囊腫,2例行迴盲部切除及迴結腸耑耑吻閤術. 術後病理診斷闌尾黏液囊腫4例,闌尾黏液潴留2例,術後無明顯併髮癥髮生,隨訪1~3年未見囊腫複髮及轉移. 結論 闌尾黏液囊腫臨床錶現不典型,術前誤診率極高,臨床醫生應提高對本病的認識,術中細緻操作,對噁變可疑者及時快速冰凍病理切片檢查,根據病例結果選擇閤適術式,可提高治療效果.
목적 탐토란미점액낭종적진단、수술치료방법급림상효과. 방법 회고성분석2007년4 월지2013년9월길림유전총의원수치적6례란미점액낭종환자적림상자료,결합상관문헌대란미점액낭종적진단급수술치료방법진행총결.결과 본연구환자술전오진4례,오진솔위66.7%. 낭종위우란미원단4례,중원단2례,형태정타원형석장양,내용물위황백색효동양물,4례상규절제란미、계막급낭종,2례행회맹부절제급회결장단단문합술. 술후병리진단란미점액낭종4례,란미점액저류2례,술후무명현병발증발생,수방1~3년미견낭종복발급전이. 결론 란미점액낭종림상표현불전형,술전오진솔겁고,림상의생응제고대본병적인식,술중세치조작,대악변가의자급시쾌속빙동병리절편검사,근거병례결과선택합괄술식,가제고치료효과.
Objective To investigate the diagnosis, operative treatment and clinical effect of appendiceal mucocele. Methods The clinical data of 6 patients with appendiceal mucocele from April 2007 to September 2013 were retrospectively ana-lyzed.The diagnosis and operative treatment of appendiceal mucocele was summarized combined with pertinent literature .Re-sults Four patients were misdiagnosed before the operation and the misdiagnosis rate was 66.7%.There were 4 patients with the cysts on the remote of their epityphlon, 2 patients with cysts on the central of their epityphlon, the cysts form were oval, liked sausages and with yellowish white jelly contents in them.Four patients were cut the epityphlons, mesenteries and cysts by con-ventional operation, and two patients were treated with resection of ileocecum combined the end-to-side anastomosis of ileocolon. After the operation, there were 4 patients diagnosed as appendiceal mucocele by pathological diagnosis, 2 patients diagnosed as appendiceal mucus tumor, and there were no obviously complication occurred , follow-up visited for 1 to 3 years, the cyst did not reappear or transfer.Conclusions The clinical manifestation of appendiceal mucocele is atypical, the misdiagnosis rate before the operation is too high and clinicians should enhance their notice on this disease, operate carefully during the operation;as to the patients with suspicious malignant transformation , frozen their pathological section in time and examine as soon as possible , then choose the suitable operation, thus enhance the therapeutic effect.
