目的 观察甲状旁腺激素( iPTH)升高程度对慢性肾衰血透患者残余肾功能造成的影响. 方法 以安阳钢铁集团有限责任公司职工总医院2010年3月至2014年3月收治的92例慢性肾衰血透患者作为研究对象,根据iPTH水平进行分组. 两组均于血液透析前及透析半年时间后检测其iPTH水平及BUN(血尿素氮)、Scr(血肌酐)、Cr(内生肌酐清除率)等肾功能指标. 结果 两组透析前的iPTH水平比较差异未见统计学意义(P>0.05),A组透析半年后iPTH水平升高为(783.5 ±68.3)ng/L,显著高于B组(P<0.05),升高幅度也显著高于B组(P<0.05). 结论 慢性肾衰退血液透析患者难以避免iPTH升高,升高幅度越大患者残余肾功能受损越严重,因此该病血液透析治疗过程中应对iPTH升高情况予以充分重视并有效控制.

Objective To observe the effect of increasing of intact parathyroid hormone on residual renal function in hemodialysis patients with chronic renal failure .Methods This article selected 92 hemodialysis patients with chronic renal failure treated in hospital during March 2010 to March 2014 as the research objects , they were all grouped according to the iPTH level .Both groups were detected iPTH levels , BUN ( blood urea nitrogen ) , Scr ( serum creatinine ) , Cr ( cre-atinine clearance) and other renal functions in pre-hemodialysis and after six months ' hemodialysis.Results The iPTH level of the two groups in pre-hemodialysis had no significant difference (P>0.05);after six months' hemodialysis, the iPTH level of group A rised to (783.5 ±68.3) ng/L, which was significant higher than that of the group B (P<0.05), and the increased extent was also significantly higher than group B ( P<0.05 ) .Conclusions The hemodialysis patients with chronic renal failure are difficult to avoid iPTH elevating .The patients who have larger increasing , their residual renal function are impaired more seriously .So, in the process of hemodialysis treatment of this disease , we should give full atten-tion to the increased extent of iPTH and effective control it .
