目的:探讨内源性肿瘤坏死因子α( TNF-α)在小儿结肠息肉中的表达及临床意义。方法采集45例结肠息肉患儿(实验组)及36例正常结肠粘膜儿童(对照组)的血清,运用夹心法酶联免疫吸附试验( ELISA)方法检测TNF-α在两组血清中的表达;免疫组化法检测TNF-α在两组患儿结肠组织中的表达,分析TNF-α的表达与结肠息肉的关系。结果 TNF-α在实验组血清中的表达高于对照组(t=5.360,P<0.05),在实验组结肠组织中表达高于对照组(χ2=10.434,P<0.05);在实验组中,腺瘤性息肉中TNF-α的表达较幼年性息肉中无明显差异(P>0.05)。结论 TNF-α在结肠息肉组织中的高表达,提示TNF-α在儿童结肠息肉的发生发展中可能起重要作用,有可能成为判断结肠息肉预后复发的标志物。

Objective To investigate the expression and clinical significance of endogenous tumor necrosis factor alpha ( TNF-α) in children with colonic polyps.Methods Serum of 45 cases ( experimental group) with colonic polyps and 36 cases ( control group) with normal colonic mucosa was detected with enzyme-linked immunosorbent assay ( ELISA ) to explore the expression of TNF-α. Immunohistochemical method was used to detect the expression of TNF-αin colonic tissue.The relationship between expression of TNF-αand colonic polyps was analyzed.Results The expression of TNF-αin serum was higher in the experimental group than in the control group (t=5.360, P<0.05), so was in colonic tissue (χ2 =10.434,P<0.05).In the experimental group the expression of TNF-αwas not significantly different between adenomatous polyp and juvenile polyp (P>0.05).Conclusion High expression of TNF-αin colon polyps suggests that TNF-αmay play an important role in the development and progression of colon polyps in children.It may be a marker for judging the prognosis of colon polyps.
