目的:探讨尿微量白蛋白(UMA)和血清胱抑素C(Cys-C)、β2-微球蛋白(β2-MG)、肌酐(SCr)、尿素(Urea)对糖尿病(DM)患者早期肾损伤的诊断价值。方法对120例糖尿病确诊患者[分为蛋白阳性组(78例)和蛋白阴性组(42例)]以及80例健康体检者(对照组)行上述五项指标测定,对检测结果进行对比分析。结果蛋白阳性组和蛋白阴性组UMA及血清Cys-C、β2-MG、SCr、Urea测定结果高于对照组(P<0.01或P<0.05),蛋白阳性组测定结果高于蛋白阴性组(P<0.05);随着患者组病情进展, UMA、Cys-C、β2-MG的检测价值明显高于SCr、Urea。结论UMA及血清Cys-C、β2-MG、SCr、Urea等对早期诊断糖尿病肾病有不同的诊断价值,医生应根据临床需要合理申请,既减轻患者看病负担,又节约医院开支。

목적:탐토뇨미량백단백(UMA)화혈청광억소C(Cys-C)、β2-미구단백(β2-MG)、기항(SCr)、뇨소(Urea)대당뇨병(DM)환자조기신손상적진단개치。방법대120례당뇨병학진환자[분위단백양성조(78례)화단백음성조(42례)]이급80례건강체검자(대조조)행상술오항지표측정,대검측결과진행대비분석。결과단백양성조화단백음성조UMA급혈청Cys-C、β2-MG、SCr、Urea측정결과고우대조조(P<0.01혹P<0.05),단백양성조측정결과고우단백음성조(P<0.05);수착환자조병정진전, UMA、Cys-C、β2-MG적검측개치명현고우SCr、Urea。결론UMA급혈청Cys-C、β2-MG、SCr、Urea등대조기진단당뇨병신병유불동적진단개치,의생응근거림상수요합리신청,기감경환자간병부담,우절약의원개지。
Objective To investigate diagnostic value of urine microalbumin (UMA), serum cystatin C (Cys-C), β2-microglobulin (β2-MG), serum creatinine (SCr), and Urea as diagnostic indicators for early renal injury in diabetes mellitus (DM) patients.Methods Detection of the five mentioned indicators was made on 120 diagnosed diabetes mellitus patients [divided into positive protein group (78 cases) and negative protein group (42 cases)] and 80 healthy people (control group), and their detection results were taken for comparative analysis. Results The positive protein group and negative protein group had higher detected levels of UMA, serum Cys-C, β2-MG, SCr, and Urea than the control group (P<0.01 orP<0.05), and the positive protein group also had higher detected levels than the negative protein group (P<0.05). Through progress of disease, UMA, Cys-C and β2-MG contained much higher diagnostic value than SCr and Urea.Conclusion UMA, serum Cys-C, β2-MG, SCr, and Urea provide different diagnostic value for early diabetic nephropathy. Rational application on the basis of clinical need by doctors is necessary for reducing patient’s burden and saving hospital cost.
