目的:研究肝动脉栓塞(TACE)治疗原发性肝癌对肝功能的影响,并分析肝损害与碘化油含量的关系。方法128例原发性肝癌患者给予 TACE 术治疗,采集术前1 d 和术后1周空腹静脉血,分析肝功能变化与碘化油之间的关系。结果术前与术后谷丙转氨酶(ALT)比较差异有统计学意义(P<0.05),术前与术后谷氨酰转肽酶(GGT)、碱性磷酸酶(ALP)及肝功能分级情况比较差异无统计学意义(P>0.05)。碘化油用量<10 ml 与11~15 ml 及16~20 ml 与>20 ml 的肝损害加重率比较,差异无统计学意义(P>0.05);碘化油用量>15 ml 与≤15 ml 的肝损害加重率比较差异有统计学意义(P<0.05)。结论 TACE 术治疗肝恶性肿瘤对肝功能有一定损伤,且在行 TACE 术治疗肝癌时要注意碘化油用量,要重视术前术后肝功能损害。

목적:연구간동맥전새(TACE)치료원발성간암대간공능적영향,병분석간손해여전화유함량적관계。방법128례원발성간암환자급여 TACE 술치료,채집술전1 d 화술후1주공복정맥혈,분석간공능변화여전화유지간적관계。결과술전여술후곡병전안매(ALT)비교차이유통계학의의(P<0.05),술전여술후곡안선전태매(GGT)、감성린산매(ALP)급간공능분급정황비교차이무통계학의의(P>0.05)。전화유용량<10 ml 여11~15 ml 급16~20 ml 여>20 ml 적간손해가중솔비교,차이무통계학의의(P>0.05);전화유용량>15 ml 여≤15 ml 적간손해가중솔비교차이유통계학의의(P<0.05)。결론 TACE 술치료간악성종류대간공능유일정손상,차재행 TACE 술치료간암시요주의전화유용량,요중시술전술후간공능손해。
Objective To research influence by transcatheter arterial chemoembolization (TACE) on liver function in the treatment of primary liver cancer, and to analyze relationship between hepatic lesion and content of iodinated oil. Methods There were 128 patients with primary liver cancer receiving TACE for treatment. Their fasting venous blood in 1 d before operation and 1 week after operation were taken for analysis of relationship between liver function and iodinated oil. Results The difference of alanine aminotransferase (ALT) before and after operation had statistical significance (P<0.05). The differences of gamma-glutamyl transpeptidase (GGT), alkaline phosphatase (ALP), and liver function classification before and after operation had no statistical significance (P>0.05). Exacerbation rate of hepatic lesion by contents of iodinated oil as <10 ml and 11~15 ml had no statistically significant difference with those as 16~20 ml and >20 ml (P>0.05). Contents of iodinated oil as >15 ml and ≤ 15 ml had statistically significant difference in exacerbation rate of hepatic lesion (P<0.05). Conclusion TACE will provide certain damage to liver function in treating primary liver cancer, and it is essential to pay attention to content of iodinated oil during TACE, preoperative and postoperative liver function damage.
