目的：探讨卡前列素氨丁三醇注射液联合Foleys导尿管宫腔填塞在宫缩乏力性产后出血中的应用价值。方法选取2013年3月~2015年3月我院产科收治的90例宫缩乏力性产后出血患者作为研究对象,采用随机数字表法将其分为研究组和对照组,每组45例,研究组采用卡前列素氨丁三醇注射液联合Foleys导尿管宫腔填塞治疗,对照组采用常规止血治疗,观察比较两组患者术中、产后出血量,血红蛋白改变情况,止血、住院时间及治疗有效率。结果研究组术中、产后2 h、产后24 h出血量均少于对照组(P<0.05)；研究组血红蛋白产前、产后无明显下降,优于对照组(P<0.05)；研究组止血时间、住院时间分别为(8.6±1.3)min、(22.6±1.3)d,均短于对照组的(22.5±3.2)min、(7.8±1.7)d(P<0.05)；研究组治疗有效率为93.3%,明显高于对照组的75.6%(P<0.05)。结论卡前列素氨丁三醇注射液联合Foleys导尿管宫腔填塞治疗宫缩乏力性产后出血效果确切,可有效减少患者术后出血量,缩短住院时间,提高治疗有效率。
목적：탐토잡전렬소안정삼순주사액연합Foleys도뇨관궁강전새재궁축핍력성산후출혈중적응용개치。방법선취2013년3월~2015년3월아원산과수치적90례궁축핍력성산후출혈환자작위연구대상,채용수궤수자표법장기분위연구조화대조조,매조45례,연구조채용잡전렬소안정삼순주사액연합Foleys도뇨관궁강전새치료,대조조채용상규지혈치료,관찰비교량조환자술중、산후출혈량,혈홍단백개변정황,지혈、주원시간급치료유효솔。결과연구조술중、산후2 h、산후24 h출혈량균소우대조조(P<0.05)；연구조혈홍단백산전、산후무명현하강,우우대조조(P<0.05)；연구조지혈시간、주원시간분별위(8.6±1.3)min、(22.6±1.3)d,균단우대조조적(22.5±3.2)min、(7.8±1.7)d(P<0.05)；연구조치료유효솔위93.3%,명현고우대조조적75.6%(P<0.05)。결론잡전렬소안정삼순주사액연합Foleys도뇨관궁강전새치료궁축핍력성산후출혈효과학절,가유효감소환자술후출혈량,축단주원시간,제고치료유효솔。
Objective To explore the application value of carboprost trometamole injection combined with Foleys catheter for uterine packing in postpartum hemorrhage caused by uterine inertia. Methods From March 2013 to March 2015,90 cases of postpartum hemorrhage due to uterine inertia admitted into our obstetric department of our hospital were selected as research objects.By random number table,they were evenly divided into research group and control group in random.In the research group,carboprost trometamole injection combined with Foleys catheter for uterine packing was applied,while in the control group,conventional hemostasis was used.The intraoperative and postpartum amount of bleeding,changes of hemoglobin,stanching bleeding time,hospital stay,and effective rate of treatment were comprehensively compared between two groups. Results In the research group,the amount of bleeding during surgery, 2 h and 24 h after delivery was less than that of control group respectively (P<0.05).There was no obvious decrease in hemoglobin before and after delivery,but the change of hemoglobin in observation group was superior to that in control group(P<0.05).The time of stanching bleeding and hospital stay in research group was(8.6±1.3) min and(22.6±1.3)d re-spectively,shorter than that of control group (22.5±3.2) min and (7.8±1.7) d respectively (P<0.05).The effective rate of treatment in research group was 93.3%,much higher than that in control group accounting for 75.6%(P<0.05). Conclu-sion Carboprost trometamole injection combined with Foleys catheter for uterine packing can obtain a definite effect on treating postpartum hemorrhage due to uterine inertia,which effectively reduce patient’s postoperative amount of bleed-ing,shorten hospital stay,and improve effective rate of treatment.
