目的：分析基础胰岛素在延缓糖尿病肾病进展中的作用。方法选取2013年4月~2015年1月在我院就诊的200例糖尿病患者，按照治疗方式的不同分为治疗组和对照组，各100例。治疗组采取地特胰岛素治疗，对照组采取精蛋白生物合成人胰岛素注射液治疗，对比两组患者的治疗效果。结果治疗后，两组患者空腹血糖、餐后2 h血糖、糖化血红蛋白较治疗前明显降低(P<0.05)，但两组间差异无统计学意义(P>0.05)；两组患者治疗后的24 h尿蛋白、尿素氮、血肌酐轻微降低，与治疗前比较，差异无统计学意义(P>0.05)，两组间比较差异也无统计学意义(P>0.05)。治疗组低血糖发生率为1.0%，低于对照组的10.0%(P<0.05)，治疗组的住院时间及肾功能恶化发生率优于对照组(P<0.05)。结论采取基础胰岛素可有效延缓糖尿病肾病的进展，其治疗效果显著，安全性高，较精蛋白生物合成人胰岛素注射液更具有效果。
목적：분석기출이도소재연완당뇨병신병진전중적작용。방법선취2013년4월~2015년1월재아원취진적200례당뇨병환자，안조치료방식적불동분위치료조화대조조，각100례。치료조채취지특이도소치료，대조조채취정단백생물합성인이도소주사액치료，대비량조환자적치료효과。결과치료후，량조환자공복혈당、찬후2 h혈당、당화혈홍단백교치료전명현강저(P<0.05)，단량조간차이무통계학의의(P>0.05)；량조환자치료후적24 h뇨단백、뇨소담、혈기항경미강저，여치료전비교，차이무통계학의의(P>0.05)，량조간비교차이야무통계학의의(P>0.05)。치료조저혈당발생솔위1.0%，저우대조조적10.0%(P<0.05)，치료조적주원시간급신공능악화발생솔우우대조조(P<0.05)。결론채취기출이도소가유효연완당뇨병신병적진전，기치료효과현저，안전성고，교정단백생물합성인이도소주사액경구유효과。
Objective To explore the basal insulin in delaying the progress of diabetic nephropathy. Methods 200 pa-tients with diabetes admitted into our hospital from April 2013 to January 2015 were selected.All patients were divided into treatment group (n=100) and control group (n=100) by different treatment.The treatment group was given insulin de-temir,the control group was given isophane protamine biosynthetic human insulin.The efficacy in two groups was com-pared. Results After treatment,fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin in two groups were significantly lower than those of before treatment (P<0.05),but there had no significant difference between two groups (P>0.05).24 h urinary protein,blood urea nitrogen,quanti,serum creatinine in two groups were slightly lower, compared with before treatment,the difference was not statistically significant (P>0.05),and there had no significant dif-ference between two groups too (P>0.05).The incidence of hypoglycemia in treatment group was 1.0%,which was lower than that 10.0% of control group (P<0.05).The inpatient time and rate of deterioration of renal function in treatment group were significantly better than those of control group (P<0.05). Conclusion The basal insulin can be effective in delaying the progress of diabetic nephropathy,and insulin detemir is better and safer than isophane protamine biosyn-thetic human insulin in treating diabetic nephropathy.
