目的 研究不同部位注射碘对比剂对多层螺旋CT血管成像( CTA)颈部动脉造影检查图像质量的影响. 方法 连续选择颈部CTA检查的患者60例,按照随机数字表法分为两组,试验组( n=30)于右侧肘前静脉建立静脉通道,对照组(n=30)于左侧肘前静脉建立静脉通道. 比较两组不同注射部位对比剂对颈部CTA图像质量的影响,使用5分制对颈动脉CT值、静脉硬化伪影、静脉硬化伪影对颈动脉的影响情况进行评价并行统计学分析. 结果 试验组外周静脉CT值低于对照组,两组比较差异有统计学意义(t=28. 4,P<0. 05). 试验组图像评价平均得分为(4. 5 ± 0. 8)分、对照组为(3. 1 ± 1. 3)分,两组比较差异有统计学意义(Z=4. 2,P<0. 01). 结论 颈部CTA静脉留置针的穿刺部位尽量选择右侧上肢来建立静脉通道,可减少静脉伪影的产生,提高颈部CTA的图像质量.

Objective To study the effects of iodine contrast agent injected at different positions on picture quality inspection of multi slice spiral CTA carotid artery angiography. Methods A total of 60 consecutive patents with neck CTA examination were selected and randomly divided into experimental group (n=30, built up venous access at right antecubital vein) and control group (n=30, built up venous access at left antecubital vein ) . We compared and statistically analyzed different injected positions impacting on the picture quality of neck CTA in two groups, utilized 5-point scale to assess the value of neck CT, intravenous hardening artifact, and effects of intravenous hardening artifact impacting on carotid artery. Results The value of peripheral vein CT in the experimental group was lower than that of the control group (t=28. 4,P<0. 05). The image evaluation of experimental group acquired average score (4. 5 ± 0. 8) and (3. 1 ± 1. 3) in the control group (Z=4. 2,P<0. 01). Conclusions CTA neck intravenous catheter puncture site should try to choose the right side of the upper extremity to establish intravenous access, so as to reduce venous generated artifacts and improve neck CTA image quality.
