目的 观察重组人生长激素对人工血管内皮化的影响.方法 对28条北京地区杂种犬进行腹主动脉人工血管移植,术后随机分为实验组(E组,n=14)和对照组(C组,n=14),E组自术后当天起重组人生长激素(rhGH)0.1 IU/(kg·d),连续两周定时皮下注射,C组术后不予注射rhGH,余处理相同.分别于术后第1、2、3个月获取标本,术后1个月E组和C组各取2条杂交犬(分别为E1m组n=2,C1m组n=2),术后2、3个月各取6只杂交犬(分别为E2m组n=6,E3m组n=6和C2m组n=6,C3m组n=6),取移植物通过苏木素-伊红(HE)染色、光镜、透射电镜、扫描电镜等方法观察超微结构变化,测定并比较两组内皮细胞数目和内膜厚度.结果 (1)术后1个月:电镜下两组材料内表面有纤维膜覆盖,均可见少量散在梭形细胞,但E1m组内皮细胞超微结构发育较对照组完善.(2)术后2个月:E2m组梭形细胞完全覆盖材料内表面,排列紧密,细胞结构发育成熟,细胞间连接紧密;而C2m组梭形细胞散在分布,连接不紧密,内皮细胞足突少,胞质水肿,核质疏散,材料内表面大量纤维组织增生.(3)术后3个月:E3m组内皮下可见少量的纤维细胞和平滑肌细胞增生,C3m组内膜明显增厚,材料表面血栓细胞和纤维细胞呈"烂棉絮"样覆盖.(4)计算机图像分析结果:E2m和E3m内膜厚度均显著小于对照组(P<0.01).内皮细胞计数E2m组内明显多于C2m组(P<0.01);E3m和C3m差异无统计学意义(P>0.05).结论 rhGH对内皮细胞增殖、减轻内膜增生、促内皮细胞成熟有一定作用.

목적 관찰중조인생장격소대인공혈관내피화적영향.방법 대28조북경지구잡충견진행복주동맥인공혈관이식,술후수궤분위실험조(E조,n=14)화대조조(C조,n=14),E조자술후당천기중조인생장격소(rhGH)0.1 IU/(kg·d),련속량주정시피하주사,C조술후불여주사rhGH,여처리상동.분별우술후제1、2、3개월획취표본,술후1개월E조화C조각취2조잡교견(분별위E1m조n=2,C1m조n=2),술후2、3개월각취6지잡교견(분별위E2m조n=6,E3m조n=6화C2m조n=6,C3m조n=6),취이식물통과소목소-이홍(HE)염색、광경、투사전경、소묘전경등방법관찰초미결구변화,측정병비교량조내피세포수목화내막후도.결과 (1)술후1개월:전경하량조재료내표면유섬유막복개,균가견소량산재사형세포,단E1m조내피세포초미결구발육교대조조완선.(2)술후2개월:E2m조사형세포완전복개재료내표면,배렬긴밀,세포결구발육성숙,세포간련접긴밀;이C2m조사형세포산재분포,련접불긴밀,내피세포족돌소,포질수종,핵질소산,재료내표면대량섬유조직증생.(3)술후3개월:E3m조내피하가견소량적섬유세포화평활기세포증생,C3m조내막명현증후,재료표면혈전세포화섬유세포정"란면서"양복개.(4)계산궤도상분석결과:E2m화E3m내막후도균현저소우대조조(P<0.01).내피세포계수E2m조내명현다우C2m조(P<0.01);E3m화C3m차이무통계학의의(P>0.05).결론 rhGH대내피세포증식、감경내막증생、촉내피세포성숙유일정작용.
Objective To explore the effectof recombinanhuman growth hormone (rhGH) on endothelialization of vasculaprosthesis.Method28 mongrel dogin Beijing arewere divided randomly into the experiment' group (group E, n =14) and the contrast' group (group C, n =14).All animal' abdominal aortwere replaced with the expanded polytetra-fluoroethylene (e-PTFE).Since the day while the operation ended, the dogin group E were the same athose in group E.The dogwere executed 1 month, 2 months, and 3 monthafteoperation respectively (group E1m n =2, group E2m n =6, group E3m n =6, group C1m n =2, group C2m n =6, group C3m n =6).Then we observed the change of endomembrane ultramicrostructure of graftand determined the thicknesof artificial vasculaendomembrane and accounted endothelial cellby meanof HE staining, lighmicroscope, transmission electron microscope and scanning electron microscope.Result(1) Fibroumembrane coveron internal surface of graftin group E1m and C1m, where we can find small amountof spindle cells.Buthe ultramicrostructure formation of endomembrane in group E1m imore consummate than thain group C1m.(2) There icompleted endothelial layecovered the entire internal surface of graftin group E2m.The endothelial cellare matured and conjunctightly.Buthe endothelial cellin group C2m are distributed irregulatedly and conjuncuntightly.Few fooprocesseof endothelial cellcan be observed, periplasidropsy, caryoplasm iscattered.There iloof fibroplasion internal surface of graftin group C2m.(3) There are littefibrocyteand smooth muscle cellundeendothelium in group E3m.Endomembrane of graftin group C3m are obviously thickening, thrombocyteand fibrocytecovethe internal surface of graftlike fluffed cotton.(4) Computeimage analysiresults: the thicknesof Endomembrane in group E2m and E3m are obviously thinnethan those in group C2m and C3m respectively (P <0.01).The numbeof endothelial cellin group E2m imore than thain group C2m (P <0.0l).There ino significandifference between group E3m and C3m on endothelial cell counting.Conclusion Administration of rhGH may improve endothelial cell multiplication and maturity, and relieve accrementition of endomembrane.
