目的 探讨组织因子(TF)测定对评估全膝关节置换术后下肢深静脉血栓(DVT)形成的意义.方法 对167例全膝关节置换术患者,采用酶联免疫吸附试验(ELISA)法检测患者血浆中TF和TF途径抑制物(TFPI)水平,对其结果进行Spearman相关分析和受试者工作特征(ROC)曲线分析,利用反转录-聚合酶链反应(RT-PCR)检测术后6 h(T1)、术后12 h(T2)、术后24 h(T3)和术后48 h(T4)时TF mRNA表达.结果 Spearman相关分析显示,DVT发生与患者血浆TF、TFPI和血小板(PLT)水平呈正相关;ROC曲线分析显示,血浆TF水平预测DVT发生曲线下面积为0.794,灵敏度为75.0%,特异度为75.9%,血浆TFPI水平检测分别为0.752、80.7%和62.0%;RT-PCR结果显示,术后6h后TF mRNA表达量逐渐增加,术后24 h达最大量,术后48 h开始下降.结论 行全膝关节置换术患者血浆TF和TFPI水平与术后DVT发生密切相关.

Objective To investigate the significance of tissue factodetecting fopredicting deep vein thrombosiaftetotal knee arthroplasty.MethodThe plasmtissue facto(TF) and tissue factopathway inhibito(TFPI) levelin 167 patientundergoing total knee replacemenwere detected by enzyme-linked immunosorbenassay (ELISA) and analyzed by Spearman correlation and receiveoperating characteristi(ROC) analysis.The expression of TF mRNwadetected by reverse transcription-polymerase chain reaction (RT-PCR).ResultSpearman correlation analysishowed thathe occurrence of deep vein thrombosi(DVT) wapositively correlated with the levelof TF, TFPI and Platele(PLT, =0.673,0.425 and 0.481, P < 0.05 foall).ROcurve analysirevealed thathe areundethe curve wa0.794, sensitivity wa75.0%, and specificity wa75.9% by plasmTF levelpredicting the occurrence of DVT, and those by plasmTFPI levelwere 0.752, 80.7% and 62.0% , respectively.Conclusion The levelof TF and TFPI in patientundergoing total knee arthroplasty were closely associated with postoperative DVT.
