目的 探讨患者前列腺组织及血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)以及CD3的表达改变及临床意义.方法 60例因尿潴留或下尿路症状(LUTS)行经尿道前列腺切除术(TURP)的前列腺增生(BPH)患者,根据病理诊断分为单纯前列腺增生组15例和前列腺增生合并炎症组共45例,其中轻度炎症12例,中度炎症25例,重度炎症8例.所有患者中按有无尿潴留病史,分为尿潴留组20例和LUTS 40例,国际前列腺症状评分(IPSS)> 25分,为高IPSS组32例,IPSS在19 ~ 25之间为低IPSS组28例.TURP术中取前列腺组织,苏木素-伊红(HE)染色,采用免疫组织化学法及Western blot方法测定组织中TNF-α、IL-6以及CD3的表达及含量.结果 免疫组织化学染色结果显示BPH合并炎症组TNF-α、IL-6、CD3表达吸光度(A)值(0.0524±0.0400、0.033 4±0.024 0、0.036 5±0.0090)高于BPH组(0.0323 ±0.6000、0.2040±0.0160、0.022 3±0.003 0),Westem blot方法检测结果显示BPH合并炎症组TNF-α、IL-6、CD3表达相对量(0.24±0.04、0.07 ±0.04、0.33 ±0.09)高于BPH组(0.19±0.06、0.04±0.02、0.27±0.06).尿潴留组各促炎性细胞因子含量明显高于LUTS组,高IPSS评分组各促炎性细胞因子含量高于低IPSS评分组.前列腺增生症合并不同程度炎症中,以重度炎症升高最为显著.结论 炎症刺激下促炎因子表达明显增多,使前列腺上皮及间质细胞增殖,导致前列腺体积增大,促进疾病进展.

Objective To study the expression and clinical significance of proinflammatory cytokin[tumonecrosifactoalph(TNF-α t), interleukin-6 (IL-6) and CD3] in prostate tissue and serum in 60 casewhich received transurethral resection of the prostate (TURP) due to urinary retention olow urinary tracsymptom(LUTS).MethodSixty patientwith benign prostatihyperplasi(BPH) who received TURP fourinary retention oLUTwere divided into BPH group (15 cases) and BPH with prostatitigroup (45 cases).The general information wacollected.Prostate tissue waobtained from the transition zone of the urethrduring TURP, then immediately stored in the-80 ℃ condition.TNF-α,IL-6 and CD3 were detected by Western blotting in the prostate tissue.The expression of TNF-α, IL-6 and CD3 wadetected by immunohistochemistry to insure site of these in prostate tissue.Between the BPH and BPH with prostatitigroups, urinary retention and LUTgroups, ohigh International Prostate Symptom Score(IPSS) and low IPSgroups, the expression of TNF-α, IL-6, and CD3 were compared in the prostatitissue.ResultThe expression of TNF-α, IL-6, and CD3 in BPH with prostatitigroup wasignificantly highethan thain BPH group.The levelof proinflammatory cytokinin urinary retention group wasignificantly highethan thain LUTgroup.The expression of TNF-o, IL-6, and CD3 in high IPSgroup (IPS> 25) wahighethan in low IPSgroup (IPS19-25).The expression of TNF-α, IL-6, and CD3 among mild, moderate, severe inflammation in BPH group also showed significandifferences.The expression of TNF-α, IL-6, and CD3 in severe inflammation in BPH group wahighest.Conclusion TNF-o, IL-6, and CD3 levelincreased in BPH with prostatitiin local prostate tissue, and the serum TNF-α and IL-6 also increased.TNF-α and IL-6 levelalso increased in the urinary retention and high IPSscore groupin local prostate tissue and serum.Proinflammatory factors,TNF-α and IL-6, and CD3 were significantly increased in inflammatory stimuli reaction, may participate in the pathogenesiof chroniinflammation in prostate hyperplasiand wainduced by proinflammatory factors, which could be used to evaluate the moleculabiology in LUTS.
